Insanity playthrough not awarding achievement

I just completed Mass Effect 2 on insanity, keeping every member of my crew alive including Kelly Chambers.  Once I beat the last boss, watched the cutscenes, and went through the credits I was plopped back into my cabin to finish a few side missions.

I reached level 30 in this playthrough as well, so I did receive two achievements upon finishing the game.  I saved my crew so I recieved "No Person Left Behind" and also the "Power Gamer" achievement because I reached max level with one character.  I made sure to not change difficulty during the entire game, and even checked from time to time to make sure it was still on the proper setting.

I know completing such a challenge should be its own reward, but I'm upset that I won't be able to show off my feat to my friends.  I still love the game just as much the second time through, so I'll probably go through it on insanity again eventually, but I really want to make sure this bug is fixed before that happens.

Some details that may help pinpointing the error:

-Playing as a female
-Infiltrator class with Warp Ammo bonus talent
-Mid to 3/4 Paragon and Renegade scores
-Legion was not activated and therefore his loyalty mission was not completed or available
-No romance subplot was completed
-All available loyalty missions complete and crew all loyal
-All available DLC downloaded and active
-Active Internet connection connected to Xbox Live on the proper profile (TheBago).
-My  first playthrough was completed switching between Hard and Normal
-Veteran difficulty is not completed
-Skipped through the credits after about a minute since I had already sat through them once on my first playthrough.

It would be really great if I could have this achievement given to me without being forced to go through insanity a second time (It's no joke), but I have a feeling this won't get addressed and that makes me sad.  If you have an further questions, my Xbox Live ID is TheBago.  Feel free to send me a message if you have any helpful feedback or questions.  Thank you.

Are you sure you didn't switch difficulty at all? I did partway through, because I was frustrated, but then went back to a save before I changed it, reloaded, and got it when I finished the game. If you didn't switch difficulties at all, you should have got it.

same has just happened to me too - I didn´t change anything in the settings except for turning the team auto use abilities on or off for a couple of missions where I wanted more control. damn annoying to spend 30+ hours and get nothing. On a similar note I had sex with Jack (same play through) and didn´t get the romance achievement either. I feel just a little cheated.

I didn't get the achievement when I had sex with Jack either but a little later when I 'made love' with Miranda I got it...that might make the difference ;)

you can't just have the fling with Jack, it has to be the full romance just before going into endgame. as for ythe insanity achievement. You have to set it to insanity at the very first screen and leave it there. If you change it off insanity even once(even without going back into the game) you won't get it.

I'm just letting people know that even changing it after the initial setup will negate the playthrough for that achievement. If you even change it and then change it back without going back into the game it negates it.

Sounds like it's best not to even go into that part of the options then, for fear of accidentally switching the difficulty level. At least just for the Insanity run. Sounds reasonable.

So if we play a different profile not in insanity and go back to the other game, it is negated?

You should be fine if you use a different profile. that shouldn't affect the game on insanity.Posted Image

I hope to get the achievement, because the difficulty really is "Insane"! No kidding there...

Had a very similar setup to OP. Also briefly started a New Game on Normal during my Insanity run (but that should have been a different profile). My saves were "full" and the game actually crashed during that New Game forcing a reset.

Kim or any Bioware can we can get more confirmation that things like starting a new game, going into the options to change squad power usage, or skipping the credits wouldn't have affected the achievement? A very frustrating 30 hour run isn't exactly something we want to do again.

What the hell Bioware!

I Just beat the game on insanity mode today and i never got the achievement for it. This is my first time playing a mass effect game all the way through and i had it set to Insanity mode from the very beginning, never changed it. Hell, i hardly even paused the damn game!

I did every side quest and all the loyalty missions and i still didn't get it!!!

You guys need to patch the freaking game ASAP!

A little update for anyone who cares:

I just finished my third playthrough and finally got my insanity cheevo! I guess there's really no point going into the details of my playthrough because of all the variables, but what the hell! :)

-Started at insanity with the new game plus option directly picking up after my second playthrough

- Saved all members of the crew (including Kelly)

-All loyalty missons complete (including Legion)

-Very little paragon this time, had a full renegade bar

-Let the illusive man keep the reaper technology

-Had Morinth instead of Samara (still no romance subplot completion)

If there is a bug, I hope maybe some of this stuff will help. I guess I shouldn't be so quick to rule out my own error, but I am pretty anal about this sort of stuff usually.

Also, thank you Bioware for such an AMAZING game. Three times through and all I want to do is play more Mass Effect! All three times I got the same shiver down my spine as my Shepard did the slow-mo leap into the Normandy. It's truly a masterpiece and I can't describe in words how excited I am for the third one! Love you guys, keep it up!

yay! finally got the Insanity achievement after playing through with a completely new character.

I hit lvl 30 after Legions loyalty mission before going through the Omega 4 Portal.

Did all side missions including the mini "quests" (Krogan Sushi, ID cards etc.)

Romanced Miranda (love the lacy bra)

Got the Insane Achievement before the credits rolled.

If you ask me insane is not really that hard (at least as a soldier)

inferno ammo FTW.

I just started an insanity play through using a character I had just finished playing on veteran the first time through. Once I imported her through the ME2 character import I set it to insanity and began the game.

Could this negate the achievement?

I did not initially recieve the power gamer achievement upon beating the game, even though i'd reached 30 by the end. I had to start a new game with it and continue to play to get it. It did not register as a "achievement unlocked" on screen, but I did end up getting it.

I have played ME2 on Insanity 3 times and last time did from before character creation and got the PC award. I have played through at least 6 times including creating a new character 2 times and haven't had the "PC" - Power Gamer (1 character through 30 levels) work or even start to initialize. Frustrated.
- I have tried importing ME1 & ME2 characters but figured that wouldn't work. Game still fun.
- Started New character creation (no import) & gets to Level 1 before I even get to meet Jacob. PC vs Xbox awards  ??? I don't know.  I have all Achievements except for this one.
- I started looking online and then posted here after the 3rd new start and getting to level 6 with both Miranda & Jacob and it doesn't register when I look. I will play it through to the end again to see if it does it.  I have PC Collectors edition with all DLC packs to date including latest visors and Katsumi.
- I heard there was a PC patch and another DLC happening in June 2010.

I started a a new character from importing a ME1 save game, set the difficulty on insanity before I did that, and left it there when it asked me what difficulty I wanted for this game. I never even looked at the options screen until I did not get my achievement and it was still set to insanity.I completed all the loyalty quests, I completed the romance plot, I explored every galaxy to 100%, and I bought/obtained every single upgrade in the game.

Do you have to complete all side quests / loyaltu missions for this? I have already done them all a bunch of times, I was hoping to just burn through on insanity and skip all the side missions, and let my crew die. Its the only achievement I dont have, so I wanted to make it a speed run
