^ I dont think you can skip all missions...I think if no one is loyal to you...then even you die yourself...so you wouldnt get an achievment
I am fairly sure beating the game plus doing all recruit and loyalty missions will get you it.

You definitely do not have to do all loyalty missions to get the achievement, you just have to defeat the Human-Reaper. I just did it with only Legion, Grunt, and Jack surviving the suicide mission. I think the only loyalty missions I'd completed were Kasumi, Miranda, Jack, and Thane.
I can't personally confirm this but I believe even the "Shepard dies" ending will still get you the achievement.
Also, I had previously only completed the game on Casual so you don't need to beat any other difficulties before Insanity.

I have a quick question.
I have been playing on Insanity up until the final battle in the DLC of Overlord.
Since I wasn't able to finish off the VR in the DLC I changed it to normal difficulty. Then changed it back to Insanity when I got back on the ship. Will this keep me from getting the Insanity achievement?
Also, when I decided to have another go at the battle I loaded the original autosave where I didn't change the difficulty. I died and chose the resume option to reload the battle. Instead it took me to the bridge of the Normandy after the battle and the difficulty was set on Insane. Is this a glitch? If so, can I still get the achievement if I finish the game?

hmmm I got the insanity on my first try, but what I didn't get is the lvl 30 with one character achievement... I assure you my lvl is 30 =/

Oh god, I hope this dosen't happen to me.
I've just spent 30+ hours trying to get this achievment.
Part way through I started a new game on a lower difficulty.
I'm pretty sure that when I went back to my original game, the difficulty was still set lower!
This has got to be the worst menu design ever, if that is the case - patch it asap - it will be soul crushing not to get the insanity achievment!

Same thing happened to me as well.
During my playtrough, I was playing two characters simultaneously. When I played my non-insanity character, I switched the difficulty once or twice, but I'm sure I never switched my insane playtrough difficulty though.
Maybe that switching the difficulty in other career caused not getting that achievemt. It is unfair, but I don't have any other explanation for this. 40+ hours on insane has been lost in vain...
God, this should be patched or something :-(

hey everyone, i had the same problem, it was my second playthrough and i completed the suicide mission and kept everyone alive and was connected to xbl. I got the achievement for completing 2 playthroughs (Long Service Medal) but not the Insanity achievement however after I got the long service medal I got disconnected from live so restarted my xbox with the internet disconnected. So I loaded my last save in the mission, from where the biotic must hold a shield barrier, then finished the mission again with everyone alive and I got my Insanity achievement. Hope this helps anyone, if you know you havent changed the difficulty then it should give it to you

Quick question. I just started another play through on NG+ importing my Lvl 30 Solder to play on Insanity. I set the Difficulty to Insanity at the beginning of the game when it has you do the game set up after the intro cut scene. Is that all I need to do for the insanity achievement (and beat the game)? Or should I have sent the difficulty level in the home screen before I started the new character?
I am about 4 hours into this play through so I'd rather start over now If I need to.
Also I changed the Subtitles setting to "Off" because I accidentally turned them on in the initial set up. That should not affect the achievement correct?

Choosing the difficulty level at the point you mentioned should be fine, though personally I would feel safer (so the speak) if I had set it in the main menu screen.
Changing other options shouldn't effect the difficulty level and the obtainment of the achievement. Just be aware that some of the dlc missions may change the difficulty level.

well. finished game today. didnt get achievement aswell. i have set difficulty before starting game and didnt even open settings while was playing. didnt import any character. Completed all side quests and all DLC quests. May be it's because i got another 2 achievements when finished game? it were No One Left Behind and Long Service Medal.
Really thinking to play once again, but do not want to fail like this

As long as you didn't change it to something other than Insanity and save it you should be fine.

I'm having trouble with this as well. I recruited everyone, but I think I left Grunt in the tank and ejected Legion. Would this effect anything?

Just to clarify to those curious about how to get it.
I actually accidentally changed it down to Casual level as I was going through the menus. I quickly noticed though since the enemies lost their extra difficulties such as Barriers/Armor etc. So, as I was going for the achievement I was really worried, then I changed it back to Insanity, and restarted the mission (the mission in question was the one that you recruit Samara). After that, I continued playing, hoping that I would get the achievement.
But tonight, I finished my 3rd playthrough to get ready for my ME3 class, and I was praying to get the achievement. And I did!!! To those folks who are worrying cause they changed it accidentally to another difficulty level: If you notice it like I did (which should be easy as enemies are probably going to lose their extra powers), You can just change it back, and restart that mission. If you're on Normandy, I don't know exactly but I think you should change it back and THEN save, or then start a mission.
Oh, I recruited everyone, made everyone loyal, and everyone survived the suicide mission. Hope this helps!

i just complete the ME2 /w ME1 imported character, play only in insane (im checking constantly when it seems too easy) and nothing, no achievement F***!
