I had no ship upgrades and no loyalty for any characters on my Insanity playthrough yet my Shepard survived for some reason. Does anyone know how to get them to die?
Is it possible to die at the end of this game?

It's nigh impossible. I've tried very hard to fail the suicide mission, and I still survived. I think the trick is, beyond have no loyalty or upgrades, you have to be missing 1-2 squad mates.
