No matter how many deep mushrooms I use, no matter how long I wait, Leliana's staminal does not regenerate!

If the stamina is part of the way to full and won't go any further, it means that she is using a sustained ability, like a bard song. There should be something subtlety glowing in the quick bar, click on that and she will stop singing and her stamina will go to the max. On the other hand it something useful to have her do, she just has less stamina for other things. It would be turned on by her tactics if you didn't do it.

Two possibilities: Either she's under the influence of a spell or so that won't let her regenerate (small red squares above the shortcut bar), or she has some "sustained" abilities that are active (small green squares above the shortcut bar), such as her bard songs or "suppressing fire" etc. These limit stamina regeneration up to a lower level than 100%.

You have to wait for the red squares to go away and/or you have to deactivate the sustained abilities.

These are my guesses.
