loyalty issues with jack and Miranda

I have a fully maxed out paragon meter but when jack and Miranda have the fight I can't seem to be able to select the paragon resolution and retain both their loyalties. However I was able to on my first playthroughs but it won't let me now on my other playthroughs. . . I mean come on what's with that

I have the same problem, the first play through I sided with jack and was able to get miranda's loyalty back with not even full paragon, but my paragon level this second time round on insanity apparently is not high enough even though it is full from what I can tell. I've also done all the side missions and possibly have talked to anyone that would affect my paragon level with the death mask.
Also if there is nothing I can do being that jack is the only one not loyal is there a way I can keep her alive through the suicide mission?
