I did every single loyalty mission, used Tali for the vents and Garrus as the team leader. When we got to the "door opening and then closing" scene, Tali took a grenade to the face. No body else died. Why?

Are you sure she was loyal. Did you resolve the confrontation between her and legion?

Did you do all the upgrades you could? How did the Legion/Tali fight turn out.

That's weird, if Garrus was team leader she should not died.

However did you upgraded thier skills and weapons?

Maybe there lays key for this events?.

On my second play through I had loyal Tali go through the vents and she came out okay. One thing though, I had loyal Miranda leading the second group instead of Garrus.

Garrus wasn't loyal. In the first segment, a "normal" loyalty rating fire team leader gets the tech killed, no matter if the tech is loyal or not.

OR you pissed the quarian off earlier by siding with the geth.

Sometimes I've Found that sometimes a character will die for no reason what-so-ever. For instance i sent legion in through the tubes and we're closing that damn door and he's gone. i have loyalty and everything. it's very confusing. And yes i also resolved the fight between tali and him,positive for both.

Had assumed the OP was referring to the door scene following the biotic bubble walk.

The fire team leader is the key for the tech vent crawler's survival.

If the fire team leader is not loyal or someone ideal for the job (Miranda, Garrus, Zaeed, or Jacob), it doesn't matter who the tech is. That squaddie will die.

if the tech expert is not a tech expert, he will take longer opening the door.

I liked Tali too much to send her into that dangerous vent. I got a robot for that.

I did a run-through with Tali through the vents and she acted like she didn't know what the hell she was doing, saying the door was stuck. Then she got hit in the head with a rocket and died . Maybe because I was pursuing a romance with her and then realized I still wanted Ashley and broke it off with her. Could that be the reason? Anyway I redid that part with Legion through the vents and no one died. All members were fully loyal, by the way.

Just corrected that one for you.  Zaeed is not an ideal squad leader.

If you trigger the final vent release before all the Collectors in the room are dead then the techie dies. At least that is what some folk have reported.
