I've played through this game 4 or 5 times now, and I just noticed that I never once received my squad achievements.  I played through almost the entire game with Garrus and Wrex in one play through and Tali and Liara in another.  Why didn't I get the achievements?

These achievements are easy to screw up. You have to bring the squadmates to as much missions and assignments as possible. Skipping one or two might be possible (but not Noveria, strangley it contains 10 of the 19 story missions), but you really have to focus on the squadmate in question.

You need to do the majority of the missions and assignments with them. Given that you have not receieve those achievements, I assume you have not done enough assignments and missions with the specific characters. Visit the mass effect wiki, it has a guide on the completionist achievement and ally achievements.

With each of those achievements it's best not to swap the pairing out until you've got the achievement!

And in Liara's case it's best to find her fast after leaving the Citadel!
