I had read that as long as you change the difficulty to insanity before you activate the beacon on Eden Prime you will get the achievement... So I started a new game with a lvl 59 Shepard and as soon as I touched down on Eden Prime I set the difficulty to insanity and ran through the game. I beat it but didn't get the Medal of Valor achievement.

I did Feros, Noveria and Virmire, then went to Ilos, then to the Citadel to battle Saren/Soveriegn. I took just about every shortcut I could, I mashed X through every convo, talked to as few people as possible, and ignored all enemies I possibly could. For example any time I had access to the Mako I would just drive by any enemies I would encounter, ignoring them. A few times during a mission I would just sprint through enemy geth and run away. There was no real need to kill any of them, I reached lvl 60 shortly after Eden Prime and my credits are maxed out so I wasn't going to get anything for wasting time killing things...
But as far as I can tell I beat the game on insanity. I DID kill Soveriegn...

I waited through the end scene and skipped the credits and never got the achievement.

I'm not too broken up about it really, just wondering if I did something wrong... Hoping you don't have to start a NEW character at lvl. 1 on insanity to get the achievement...

You have to change the difficulty before you touch down on Eden Prime, as in the Normandy before talking to Anderson and Nihlus.

I thought the first time you were able to access the menus was right when you land on Eden Prime?! Looks like I'll have to do it all over again. :/
