All ME2, all ME1 achievements (I went back to finish ME1 with a level 60 character - I'd reached level 60 but not fnished that playthrough with my preferred paragon choices - and picked up those training simulation achievments from the DLC I hadn't bothered with before).
In ME2 gettng the insanity achievement was the only one that I recall as being particularly difficult the first time, at least. If you can get that achievement, I can't see having real problems with any other achievement. Even having everyone survive was just a matter of (when on my first playthrough Mordin died) checking to see what I'd done "wrong" (in terms of assignments - I thought at first that character died because I had unspent skill points). Nothing to do with "ability" to play the game. Some of the achievements like brawler (?) just seemed a bit silly to me, I got them for the sake of completeness but nothing to do with my playstyle. I just replayed the very beginning (after Shepard wakes up) to punch and shoot a few robots. Reload and repeat until 20 killed after being knocked back. Heavy weapons achievement, kill with one weapon, reload and choose different weapon, kill, reload...etc.
Some of the ME1 achievements were I thought unduly onerous (e.g. getting some of the "ally" bonuses meant you had to skip quests until recruiting the character), but felt more like an "achievement". Killing the thresher maw on insanity felt like a real achievement to me, but I already had that achievement from my first "normal" playthrough...
On that note, if it were possible (maybe in DLC?) to have a few difficulty-based achievements that would be nice. For instance, no achievement for completing a mission, but only for completing it on hardcore or insanity or something (vis-a-vis, not an achievement, but you can only get the Geth pulse rifle on hardcore or insanity).

*signed* 4 play through's, 3 i plan to import to ME3, all 51 achievements. loved every minute of every play through.....OK the times fighting scions/husk swarms on insanity pissed me off but that was easily remedied with a shot from the trusty Cain.
