scions go down real fast with inferno/incendiary ammo and the viper sniper rifle.

Heavy mechs can be annoying, but theres certain positions you can take that get the robots stuck on insanity. Praetorians are sorta similar that way.
Overload or any flame attack on a flamethrower = dead flamethrower.

Any enemy who can hit me behind cover, or can forcibly make me leave cover. The amount of times I died on Insanity because Harbinger blasted me away from my cover...
By the endgame, whenever Harbinger or a Scion was nearing me, I'd whip out my Tempest with Inferno rounds, strafe around him, and the moment my shields went down, charge that SOB. Worked way too well.
But, to give a concrete answer to your question: either Harbinger, of maybe those pesky Abominations.

Geth Hunters, since my NPC buddies don't see them as they bum rush me with shotguns, the next would be geth primes since they are insane with their reaver machine guns.

Heavy Mechs were a pain in the beginning but the overall honor of most annoying has to go to Scions. When Heavy Weapons do more damage to the final boss than to Scions, there's a problem.
I never had trouble with Harbringer, I was an adept with Miranda and Mordin in my team and Harbringer would usually be dead less than 2 seconds after his feet touched the ground. Warp, Warp, Incinerate, Warp = Dead.

Is the Scion attack ignoring cover only on Insanity? Never had a problem avoiding that cannon wave on Hardcore. I have more problems with Harbingers.. that slow moving energy ball will knock you back unless you are behind heavy cover.

Scions for sure. Attack pierces in most situations, and they usually come supported by a horde or fought in a relatively confined area. Open space wouldn't be so bad since you would be able to kite/circle strafe then to death but cover ignoring attacks + support troops = pain.

Scions are the worst. I go to ridiculous lengths to take them down asap. It's frustrating when you get knocked out of cover and you can't do anything about it.

The only enemy that has annoyed me on my Infiltrator on Insanity is the Praetorian. I still haven't seen the Scion's shockwave go through cover.

Collector Guardians. The moment before your about to take them down they pop up that shield and turn into harbinger.

Husks and Scions--ESPECIALLY when paired.
It's easier to deal with them if I have Samara and Mordin along (Reave and Incinerate for the Scions, Throw Field and Neural Shockwave for the Husks), but they're still annoying.

i love fighting swarms of husks and other toughies
definetly i hate heavy mechs at the early stage of the game ( side mission when u need to secure crates and destroy 3 heavy mechs on insanity )

Scions and Krogans. The end boss for Grunt's recruitment mission was possibly one of the hardest encounters on insanity for me so far. Only reason I won was because the Heavy Mech bugged out in the back and didn't move.

Husks.... I hate them so much, ever since Eden Prime. I hate them especially when my squad mates turn into idiots and stand out in the open, get killed by the Scions and the 20 Husks all come running at me.

Human Larva........completely ruined the feel of the game for me, It's felt liek a bad boss fight at the end of some bad nintendo game.......and Scions ton's of hate, BUT I LUV ME MY VARREN! so awesome wish i could keep one as a pet that could kill things!

I hate that Krogan Warlord on Jack's loyalty mission.
He has an affinity to laugh at all of my puny bullets, and then run into me until I die.
