You are not alone. She reminds me of the "not as insightful as you'd hope" psych majors I've met.

The reason he was 'brained' was because you made Thane your 2nd squad leader. He's not a leader, in the least.
My first playthrough I made the same mistake. Legion got shot, then I redid it with Tali and so did she. I couldn't figure out why, as both were loyal (I had enough Paragon to convince them to co-operate). But it turned out that my choice of Samara as the 2nd squad leader was bad, even though she had 1000 years of experience and Miranda actually congratulated me on choosing her.
You need with Garrus, Miranda, or Jacob to lead the 2nd squad both times for no one to die. And they need to be loyal.
Hope that helps.

I did not do Legion's and Thane's loyalty missions and yet they still lived...I don't get it.

GT : Straying Bullet
The dates will show it's one smooth proces.

Jacob/Miranda/Garrus as squad leaders.
Jack or Samara as second specialist.
(Suggest taking Mordin with you on your final squad, but not sure if it's necessary or not.)

Yes, you can save your crew but you have to go almost immediately after they're abducted. I did IFF, then Legion's loyalty mission, then hit the Collector base -- I got Kelly and the rest of the crew out.CHULK88 wrote...
Just completed the game entire squad is alive but the kelly woman died in the pod is it possible to save her?
I saved everyone 1st time in fact, I was so proud of myself. I was biting my nails after every decision, omg. I would have been really sad to lose anyone; I honestly like all the characters, even the ones I thought I wouldn't like when I first met them.

Did this and sent Mordin back with the crew. Saved everyone including the entire crew on the first playthrough. I believe you can do one mission and one mission only before immediately going to the omega 4 relay after the crew is kidnapped. The longer you wait the more crew members will die. I went immediately after the crew kidnap.

In my game, my paragon femshep saved everyone but Legion. I had all loyalty missions except Zaeed. I had Legions loyalty but in his confrontation with Tali, I took her side and told Legion not to send the data on the Migrant Fleet. I assume this is where I went wrong....maybe should have taken the middle choice. I sent Legion into the pipe and Tali got shot, but her shields held. Legion got brained at the door. I saved Kelly and Chakwas and the crew. Just wondering if anyone had saved everyone and got the achievement? How did your game turn out? Could anyone save everyone with a renegade?

If you side with one person, the other loses their loyalty towards you. Pretty much Legion said f**k you and died.

Team 2 Leaders (Fire, Distraction) must be picked from: Garrus, Miranda, or Jacob. These three must be loyal. The allocation order does not matter.
The Tech Specialist is most likely to die from gunfire or a stray rocket at the jammed doors if the Fire Team leader is not one of the above three characters, or if said Team Leader is not loyal.
It doesn't matter who the crew escort is, but he or she must be loyal.
Someone has said that a non-loyal Subject Zero will still survive along with your own squad of three if she is used as the Biotic specialist. *Unverified*
There seems to be a lot of speculation re: Mordin's random case of death. I have not been able to replicate this. Mordin has never died on my plays through the Suicide Mission, regardless of where I use him (or in some cases, don't use him).

What a fantastic way to prove one's defiance.

I know, everything leads you to believe Samara is a good choice, but suddenly: *Bam!* Legion's dead. I mean, Jacob's a better choice than Samara? Really?

Re: Jacob. Supposedly, being a former alliance soldier, he's had more combat experience within a team. If you pick him as Fire Team Lead, Miranda comments on this being his first time being a leader (or something to that effect).
