Shep dies if no one else but Joker is left to pull him back to Normandy. So only after loosing EVERYONE can you also kick it permanently.

I don't think any of the other squad mates or crew is around to help you. I don't know for sure, though, but my impression is that for you to survive, one of your final squad mates must survive.

Not enough, as any surviving Hold The Line crew member will also help Shepard to the ship. Someone lost both squadmates with reaper, only to have Zaeed help him up.
All have to be dead and gone for you to die.

That was actually disappointing, to be honest. My original impression from the marketing was that in cases when Shepard died, he had to sacrifice himself for the mission.
That didn't happen here. He simply dies because he's a lousy commander.
I was expecting an epic cut scene where he charges in and saves someone or blows up something, or SOMETHING.

It would be hard, but possible, you need one good loyal defender to hold the line and 2 not loyal in your squad, you should end up with only you, the defender and Joker alive.

So be a lousy a commander and the problem solves itself.Jeremy Winston wrote...

Hehe exactly.
BioWare made sure that Shepard dying is not heroic, it feels like a bad failiure ... so you go back, get your ship and crew straight and the kick some Reaper scum in the daddy bags.
Instead of being kicked that is.

Huh? The problem is that I expected Shepard to die in a blaze of glory under the right circumstance. I don't WANT to kill him off. It was just impression from Marketing materials is that what would happen.Naltair wrote...

I saved everyone, so I'm good.

But really I'm glad they went the way they did. Dying for poor leadership skills is a very fitting way to go. It's such a subtle reason to die (Joker not strong enough) that I loved it.

The death is not a complete waste, as the reason Shep dies is to save Joker's life.
But yeah, you only get to save him by dieing, one person. And after that TIM takes his ship away from him ... major suck if you ask me.

Do you have video of this? I've played around with it a little bit, and I've never seen Shep survive if the two final boss teammates die...not calling shenanigans or anything I am just curious about all the endgame mechanics.

No sadly.
I didn't do it myself, someone on these forums posted about it a while back.
