On a recent playthrough I was doing Miranda's loyalty mission. Dureing the conversation at the start of the mission I took the renegade interupt prompt, when the cut scene started Shepard was no where to be seen. The whole scene went as it normally did, correct camera angels and all, however my Shepard was invisible, he appeared fine once gameplay resumed.
