I'm playing my second playthrough in insane mode, female character imported from ME1, on Xbox 360 collector edition, Italian version.
These are the most annoying issues I've found:
- wall noclip error: sometimes Shepard walks on or through tables, tents, walls, boxes and parapets. Once the character walks in or upon these objects he gets stuck and there is no way to get him out of there. In this situation sometimes pressing A button he goes up going out of the map and I can see the 3D model in the black void. This happen in some maps. The only option is to reload the last save. Thus far I had to start over two or three battles because of this bug. This is definitively and really annoying.
- In several conversation scenes some characters disappear and reappear in different positions jerkily. It looks like a framerate problem or somethis like this. This happen almost every time breaking the mood. Really annoying.
- Sometimes the voice dub disappear, breaking the conversation.
- A few times also the companion characters got stuck in the walls or objects. Is these cases I can see their powers available in the power wheel but they don't work. The characters are like dead. Using cohesion doesn't remove the problem.
- In a few points, enemy bullets goes through walls and objects making cover useless. I got killed once because of this ghost bullets through walls. I was on Omega in the "Vent" battlle.
- "run" and "take cover" actions are controlled by A button. Sometimes when Shepard is running he get stuck in a unpredictable way because he walks close a box, a table or another object, usually under fire. And get killed. Annoying.
The games is really awesome. You did a great job guys.
Please check this issues and release a fix. Some of them really breaks the experience.

aside from the issues with 'getting stuck' I have pretty much experienced the same problems. What I also experienced in the German version is that Shepard sometimes leaves the cover when you switch to a different ammo type, etc. On insanity difficulty that can be pretty fatal and gets annoying when you have to do a lot of stuff all over again.
what I'm curious about....maybe one of the Bioware guys can answer that question....are these animation and cut scene bugs actually fixable with a patch? Just wondering if that is even possible to fix...not asking for a specific time when or if it will be fixed.

I only know that on PC version there are no "conversation jumps", but they have the wall / noclip error as well.
