Well, most of the others have been mentioned already, and it's not a "shock" moment, but I felt awful after hearing the verdict on corporal Tomb's case and receiving an email from him - my Shepard was a Sole Survivor. That man was broken in ME, I thought I gave him hope and purpose to live on... only to have his life ruined again.
I wanted to just open the comm-link and give TIM the middle finger and a prolonged session of swearing...

The colonist being.. melted on the collector base still gives me chills.
Garrus getting shot to pieces. I thought I'd stuffed up and got him killed.
The last is not so much disturbing, but it made me pause. The salarian on Illium, near the souvenier store, talking to the asari. He wants to get his asari partner a gift to remeber him by, and he says to the asari with him something along the lines of "I'm almost 35.." and "You'll make sure she keeps it, won't you?" ... The short life span they have never really hit me until that.
