Other than it being cancelled.
For me, it would be if Bioware announced that Mass Effect 3 will be an MMORPG.
God that would suck.
What is the worst possible news you could imagine hearing about Mass Effect 3...

ME to be a pure TPS now, running straight foward in corridor missions, with a good and a bad ending.
Including Now multiplayer with deatmatch, capture the flag and some horde wave.
ME2 was just the transicion ;-)

Honestly I wouldn't mind this so long as playing from a first person perspective was optional kind of like in Vampire The Masquerade:Bloodlines.
I can think of a few moments in the action where playing FPS would help.

If they kept the same Combat system for ME2. If BioWare wish to make this game more of a shooter than an RPG (not saying that they aren't both in some contradiction of the terms) they need to change the combat system.
Gears of War, Red Faction and Army of Two for examples are really great 3rd person shooters and have great game mechanics that I believe ME2 lacks. Something that needs to be really looked at for ME3.

That would be good news because Liara is so ugly...and blue.

Shepard is really truly permanently dead, and you have to play ME3 as Zaeed, who manages to survive even if you got him killed previously, because rage is a hell of an anesthetic.

Multiplayer. Multiplayer should never taint Mass Effect.

If it was dumbed down for a mainstream audience rather than staying true to the fans that made the franchise what it is today. Oops already happened, haha.
Seriously though if it became Rainbow 6 in space with light RPG elements. Dang that happened too. I cant win for losing today.
In all seriousness I really hope they take the feedback and the suggestions to heart, obviously we're all going to get ME3 when it comes out, I just hope they listen to the fans, yes even the fans that discovered ME1 just 3 whole months ago, or the fans that said NO! to actual planet exploration, or NO! to an inventory system, or NO! to anything requiring actual thought instead of just running around shooting stuff, or even the fans that take the good and bad in stride and hope for the best in the future, the ones that dare say anything from their point of view only to be told to F off and go play MW2.

1. "We won't change much, only make a few twists here and there to make the game easier accessible for a broader audience."
2. The ME 1 companions are dead / gone.
3. No PC version.

"Just so we can please everyone, ME3 will now be a full-fledged RPG in the tradition of Baldur's Gate 1 BUT will also include a full-on FPS mode that you can toggle on or off. Everyone will be romanceable - everyone. Everyone will have a face. Every region will be fully explorable, either by ATV, hovercraft, or just reading the huge codex entries for free xp. There's no level cap, and ammo is unlimited after you complete a simple sidequest. Because of this, the game will come on 25 discs, will be Xbox only, require constant online access, and cost $1500."
