I have completed the game with everyone surviving. Now I want to see the worst ending, but the last mission (after the IFF) will not trigger.
I don't want to do loyalty missions, because I don't want everyone to survive, but the mission does not come up.
Do I have to complete loyalty missions in order for the mission or what? when will it appear?

If you took Legion's mission, game waits for that. It's safe.
But you get no more than one mission, it will trigger after that one almost certanly.
