Depends on the situation, often Renegade 'chat' allows you to get more renegade options as paragon does paragon options but there are some renegade choices in all situations, and some paragon choices in all situations.

Though when it comes to getting 100% of one and 0% of the other, it might be very difficult to do everything in one way only.

I attempted to go full paragon, but ended up with 1/4 a section of the bar renegade. You can pick up a point or two here and there at various points in the game without meaning to.

No. There are some required storyline quests that give both paragon and renegade points no matter which option you choose. You just get more of the one and less of the either but never either/or.

That...and most*class* skill trees give para/ren points at an even split.

No, BioWare FORCES Renegade points at the very beginning, to make your scars appear.

You need some guide or something, since renegade/paragon is sometimes impossible to figure out in the dialogue options.
