Your squadmates do a pretty good job on Normal and Veteran. It is only on the higher difficulty settings where they will start to hamper rather than help. Here's a list of things I have noticed that might be something to look at for ME3's squad AI.

Cover: If you don't tell your squadmates to find cover, they will actively move from point to point. As we all know, if you spend more than a couple seconds out of cover on Insanity, you're dead. So watching my squadmates amble around aimlessly while getting cut down by enemy fire really is frustrating.

Solution: This one is easy. If you direct them to cover, they will stay in it.

Self Preservation: Your squadmates have no sense of keeping themselves alive. No matter how little shields or health they have, the will always pop back up to take pot shots at enemies. I have tested this and watched squad members lean out of cover without letting they're shields or health recharge. Even at critical health levels, they will still lean out and get killed.

Solution: Allow the player a set of options that includes when to remain defensive eg. In Tales of Vesperia, you could set your party members to stop attacking the enemy and run away once their health reached a pre determined point. It pains me to see that party AI in a Tales game is more manageble than in ME2 but that's how it is.

Squad members going Rambo: If you don't tell them what to do, squadies will sometimes run right into the thicket of the enemy. I have seen Mordin charge a Krogan before. He just decided to exit cover and make a beeline for death.

Solution: Make them smarter I guess.

Anyone else who has things to add to the list are welcome, of course. I hope that someone at Bioware sees this and that it might help them in creating better squadies.

AI is pretty bad, in general, but the thing that gets me most is:

When the AI tries to advance, and gets in cover where they're obviously flanked. They do not move. On any difficulty. It's painstakingly annoying to just watch them there getting shot in their sides and backs. And it's not like they just GOT flanked, that enemy was always there, and they advanced anyway...

Squad AI seems function a lot like enemy AI eg they will sometimes give the enemy something to work with. Well of course, your squad shouldn't be doing that haha.
