No. You just have to accept that at least one team member will death. It's worth it to stop those collectors, though.

Make sure all are loyal (technically don't need this but just to be safe) and make the correct choices on roles. Don't assume if someone dies in Role A you made the wrong choice there because it could be an incorrect choice for Role B that made the impact.

Yes, it's completely possible and not even very difficult. Just get every ship upgrade and personal upgrade (like Mordin's omni tool) and make sure everyone's loyal. For the special roles in the end, do it like this:
Tech: Legion or Tali (I did Tali)
Second Team Leader: I did Miranda both times.
Escort: I did Jacob, though I've heard Mordin works well too.
Biotic: Samara or Jack, I did Samara.
I don't think I left anyone out there... also, if you didn't send Mordin as the escort, I recommend taking him with you into the last fight. A lot of people have him randomly die otherwise.

Yeah. I don't remember the sequence exactly, but I'll try my best.
1. Do all personal quests and gain loyalty of all your squaddies
2. Get all the squaddie-specific upgrades before you head out.
Choose samara for the biotics decision.
Mordin for escort decision.
There are some more, but I don't quite remember. Sorry. *hops on failboat*

Squad upgrades don't matter at all.
1. Do all loyalties.
2. Purchase all combat-related ship upgrades.
Tech: Tali or Legion
Team Leaders: Miranda or Garrus
Escort: Anyone, but I've only tried (and succeeded) with Zaeed and Grunt.
Biotic: Samara and Jack.
