Thats because you forgot to throw a heavy Shockwave to scatter his buddies then pick them off one by one.
Vanguard by far is my favorite class

try this on you Vanguard
Spec in the Heavy Shockwave and the Spec Jack in Area Shockwave.
Then have her do an Area Shockwave and follow it with your heavy shockwave

I treat Charge as a finishing move against the tougher enemies. You can take them down pretty quick with Charge + close-range shotgun. As mentioned before, you have to make sure you take care of the weaker enemies first, with Shockwave or use your squad mates abilities.

How it works:
Kill the defenses of someone on one side of a group of enemies.
Dominate them.
Wait for cooldown, then charge an enemy on the opposite side of the group from the one I dominated.
This results in the group of enemies getting multiply flanked from me, my squad, and (until Dominate wears off) the poor sap who I mind-controlled. Even if they don't die quickly from my first charge and attack, they are generally distracted/weakened enough for me to either charge/shoot again and/or Dominate someone new whose defenses are down, run for cover, and repeat.
Mostly has worked out pretty well. The hardest part so far has been the Colossus area of Tali's recruitment quest (All Geth everywhere and no AI Hacking

I mean, only playing the game in one way, because I like it a lot and because I like it, I get bored when I try something else. Limits my life. Or at least playing Mass Effect.

I've just about beat the game with a Vanguard on Insanity. It's awesome to really use the class to its full potential. There's no better feeling than zipping about the battlefield and destroying your enemies one by one by one- on Insanity I can clear a section in 30 seconds that took me several minutes as an Adept. Don't die a whole bunch, but I seem to die about the same- with a Vanguard its because I don't charge the right guys in the right order (usually start with the ones furthest from the group and work your way in), with an Adept it was because I'd get flanked (by not killing the toughest/fastest guy straight out).
