I hope that the "reward" for faithful players is just that they get to rekindle their relationships, not that the ME1 romances are given more content than the ones from ME2, and a player gets punished in some way if they "cheated." My Shepard broke up with Liara the first chance that he could, which was at the end of LOTSB. Should he get in hot water just because he got together with Tali beforehand, while thinking all the while that him and Liara were "just friends" at this point? I think what happened at the end of LOTSB was more a rekindling of a romance than a direct continuation, especially since Liara was so willing to not rekindle it. Hopefully, the VS romances are dealt with the same way (maybe in DLC); they apologize for Horizon and Shepard apologizes for not finding them immediately, and if Shepard decides to get back together, s/he can.

I would presume that if Shepard does get back together with the ME1 LI without breaking it off with the ME2 LI, there will be some sort of confrontation. Maybe then, there could be a punishment of sorts (like both LIs leaving Shepard), but only if Shepard has not broken up with either LI beforehand when given the chance. It's not really fair to say that because someone romanced character A in ME1 and character B in ME2, that Shepard is a two-timer and deserves to have it rubbed in their face. It's been two years, Shepard was dead, and there's no option to reply to Ash/Kaidan's email with "Thanks, let's just be friends now."

Liara as a romance available in ME3 no matter what would make sense though, she seems flirty towards Shep in LOTSB regardless of LI status.

It's obvious that whatever "consequence" occurs as a result of cheating won't be something game-breaking or that will prevent folks from finishing the game. Mostly because Bioware can't do that. I'd lay bets that it means they'll treat Shepard differently through dialogue, depending on how the break-up is handled. Remember in ME2 when there are LI conflicts, you can choose to let down the one you don't want easily, or hard. I imagine ME3 will be the same way.

Also, I doubt they'll try to make the player feel too horrible about cheating. Liara handles the idea of you being with someone else rather well all things considered, and it's easy to keep it going with her or break it off. In addition, the confrontation with the VS made a number of friends of mine feel rather raw. And I know a few who don't think the apologetic email was good enough for them.

So I think Bioware won't make it a huge cinematic moment, but I can see them making it something that carries through to the end of the game and into whatever epilogue they choose to use.

I think that an ending with Miranda could be that you have a child with her even though in the shadow broker dlc you find out that she can't have kids.
