yes basicly the followers are so stupid most of the time
and as i see to progress to insanity - you must first upgrade much
cause the start is hard - after i believe it will straighten up
i put warp rounds ultimately to my vanguard cause all the other powers i find them
lacking in many things :S
so i empty the fire and ice bullets and i maximize all my biotic abilities
and the warp rounds (jack special ability)
Thank you Bioware for making Insanity so hard !!

There are ways to finish it without warp spam. I haven't done it as a soldier NG+ on insanity, only as a sentinel, but my approach was to completely ignore the scions. They can't hurt me behind cover, and they can't become harbinger, so they're the least of my worries by default. When the first platform drops, warp two of the collector barriers (one warp by me, one by thane or miranda - or use AR or SMG if you can't warp) and then throw or concussion shot them both over the side. On lower settings you can actually clear the platforms completely without seeing Harbinger but I could never kill more than 2 on Insanity. Once those two are done, you take out Harbinger. If you have time then you take out the right scion while you're waiting for the second platform. When the second platform arrives, you again quickly kill (warp/area reave/AR/SMG and throw or concussion shot) two of the collectors, and then the third, who's now Harbinger. Third platform arrives, I shift to the left side of the barrier (I'm still on the first platform, I never move) and clear the third platform exactly the same way as I cleared the other two. It's a bit tougher now with the assassins but still not a major problem. Then, after Harbinger, you drop the second scion and viola! you're done.
It's not really hard, it's just time-consuming, and you have to be very, very patient and very, very careful. The scions are only a problem if they shockwave you while you're out of cover, and that should never happen if you're paying attention. Harbinger won't knock you out of cover, either, if you're strafing even a tiny bit behind cover; moving throws off his singularity.
My squad usually dies pretty quickly against one of the first two platforms, and I leave them there. It's just as easy to clear the platforms without them, it just takes a little longer...

One Harbinger isn't an issue, I find it's best to cut down all the little guys first, to avoid crossfire. Lift-Warp detonation is extremely effective. Team focus fire on the weakest (Drones) first. The AI squad isn't horribly stupid, if they are taking damage to critical, they usually get down in cover, but the Scion can wipe them out from shields-down status. And it's hard for them to have any recovery time with the drones scoring weak but frequent hits.
Garrus, Grunt and Zaeed really shine here, if you have two Concussive Blast characters, you can take down the first platform's Drones in a second with a little luck and a grenade. This lets you work over the Scion before the second platform arrives, if you can take the scion out before the second platform gets there, the fight is practically won, since you have plenty of cover and some fall-back positioning.

After getting Dominate, I just have to say that I have no idea how I survived without it before. As an Adept, it is definately the way to go, if anyone asks me. Keeping enemies at bay by having them fight eachother, being able to recast Dominate before they have even destroyed the barrier the first time.. simply amazing.

I took dominate on my infiltrator, of all things, and I'm loving it. The ability to control enemies in any situation is just priceless.

i get kinda pissed at times when i have to reload... but whats insanity is the only mode when you finish a mission you feel "damn... that was good fight" and like you actually accomplished something.

ok guys after many loses hehe
i find the perfect combination for not dying in insanity
First you must change from the options the automaticly cast of power from your stupid followers
so you throw the powers whenever you like
Second for Vanguard the best ability is the warp rounds (Jack) at last you can kill with
efficiency
and third use much cover - this makes me a little unhappy cause i dont use the abilities
of vanguard in full force :S
Thats all ...
AT LAST I WILL FINISH THE ****ING INSANITY !!!!

I can't handle this anymore. I played as an adept and made it all the way to horizon. Once the husks come around it's a serious pain the ass. I can't handle all the husks rushing-in and then harbinger at the same time.
I heard it's easier with a solder so I started another career. as a solder I can't even make it pass the first encounter with the blood pack on "mordin's" recuit mission.
