I almost always try to create look like me, after all it's part where I lead in story, isn't , but for somereason, sometime, when I create, it's sure looking nice than my real face, of couse, it's may has to do with my teeth, my real teeth is full of yellow and break part.

I tried to do this in Fallout 3, Mass Effect, and Dragon Age. In Dragon Age, my guy turned out quite a bit handsomer than I am. As evidenced by my avatar.

Often my first created character is more or less looking like me when possible, but I don't always do that, it depends on my mood.
Else for other characters, I've always create them with a look which kinda matches the personality I wanna roleplay. Of course that has a meaning only for my eyes, as for sure someone else might assign another personality to them at first look.
Examples:
Evlys: young selfish, lying, and vicious human female mage:
Asys: City Female Elf Rogue, who don't care about human laws, but is still respectfull of anyone, but she won't hesitate to kick your ass if you are mean to her or other people.
Connor: Sympathic and Helpful Noble Warrior
