[Basher of Glory]

Although I played the game now with every class except the sentinel, I never lost anyone during the suicide mission. I played all games on "normal" and was always very high if not 100% paragon.
Nevertheless, I read about crew members dying.

Questions:
1) AFAIK they die only, if they are not loyal, right?
2) How can we loose the loyalty of one or more, if we are high enough paragon/renegade?
3) Does game difficulty change things, i.e. someone dies although loyal?


Minor stuff: Names of aliens.

Most of the alien races have names, which sound somewhat alien: T'Soni, Wrex, Shisk, Barla Von etc.
Only the Turians have either latin or Armenian names:
General Septimus, Saren Arterius as latin examples and Garrus Vakarian as Armenian example.
(Side note for our friends from the US: 
No, Armenia is not 50 miles south of Rosebud, Texas. It's a republic northeast of Turkey, south of Georgia and west of Azerbaijan)

Is there any explanation in the lore, why the Turian names aren't  like the names of other aliens?
