Reading around, I've noticed 2 endings that I have never managed to get:
Connor's Sister, and Shale's return to Dwarf Form. How do I arrainge for these?

I don't know for sure about Connor's sister (I'm assuming you mean Isolde having another child?), but I think it involves killing Connor during the Redcliffe quest line.
As for Shale returning to dwarf form, you have to be Friendly with her - in the 90-100 zone. If you live, Wynne goes with her to Tevinter to search for a way to revert her to a dwarf. It's not mentioned in the epilogue if they find an answer, though.
For the epilogue that implies Shale returns to a dwarf and goes around killing birds, you still have to be Friendly - but sacrifice yourself when killing the archdemon.
