No idea. If you have all loyalty, it shouldn't happen. I personally finish their loyalty quest and make sure I have done all of their conversations, Just so I concretely have their friendship or loyalty.

Try to put as many defensive characters in the hold the line group as you can. Take the wimpy characters with you or have one escort the crew back. Its all based on a combined defensive score of the characters you leave. Tali, Jack, Thane, and Mordin are the weakest defenders. Grunt, Garrus, Samara, and Zaeed are good defenders. Also, the more time you spend fighting the boss, the more are likely to die. There's no visible counter, but one is running.

Mordin and Tali aren't frontline fighters, so they have the highest chance of dying, despite being loyal. If you can, send Mordin back with the crew, and leave your heavy hitters to hold the line - that means Grunt, Zaeed, and Garrus. They'll help keep the others alive.

Never ever ever ever leave Mordin to "hold the line". Tali can survive just fine but Mordin has like an 80% fail rate there for some reason. You have to send him to escort or bring him with you to the final boss otherwise he will die.

I have a feeling the main problem is that you are taking too long on the final boss though.

Here's a good piece of advice, use the Cain on the final boss. One hit practically brings him to the brink of death. As someone else pointed out already, the faster you kill the boss, the less likely you'll have to worry about someone dying.

ok they die before the boss you kno the part where ur shooting down 4 vials that are holding it up then you get the cut scene asking for the status update with them, then the elusivie man ask you not to blow the place up. thats where i see some random person dieing. i done this 4 times. hell i even take the person that dies with me and another person dies in their place im like wtf!!! also i have seen garrus die twice and grunt once so u sure its about their toughness?

They take a hit, but are picked up iirc, they don't die. It's just a part of the scene.

This is not entirely correct. I had only 4 people holding the line: Garrus (2nd squad leader, got a hit), Miranda, Mordin and Samara (not loyal, exhausted from maintaining the barrier through the seeker swarms), and they all lived. Replayed 10 times in hopes that Mordin kicked the bucket but he wouldn't!

I think the lines about 'no matter who we've lost' and 'some of my team gave their lives' are there no matter what; I remember giving Miranda a serious WTF look when she came out with that after I'd successfully saved my vent specialist.

You always see someone get knocked down (YEAH, ON YOUR ASS /Grunt) during the Line-Holding cutscene, but don't write them off as dead until you see the coffin.

I always have Morden hold the line and in 3 full playthroughs and about 5-6 runthroughs of hold the line everyone has always lived.  I suspect some choice of dialog, not doing some upgrade, etc. is causing some of the problems.

Just make sure everyone is loyal, you have done everyone's upgrades, you took the time to level everyone up and spend their points, done as many squad updates as possible, make sure your lines are "I'm coming back" ... and you should be fine.  If you run into real problems then send Morden back with the crew and take Tali and Thane with you on the final mission and hopefully you'll be fine.

Even though most consider it cheap I'd take CAIN as the heavy weapon to make short work of the Reaper larva and hope that it helps.

are you actualy seeing a death scene? or just someone getting nocked down in the background when the one individual (always thane for me) is talking to you on the radio? i always see someone getting nocked down at the point but i never have anyone die on me.

Don't worry about who dies. I think it enhances the realism. Thane died in my first playthrough and even though I took a risk choosing him, I have no regrets. I'll make the same decision in future. Bioware won't screw players that lost a few squad mates.

Eh, I had Garrus going down in that short intermission in the final fight, but he was fine when I got back to the ship.

You know, I've never lost any squad mates, and I've never sent Mordin back as the escort. Huh.

There was an offical summary of how Hold the Line worked.. and I don't recall it depending on how long it took to kill the boss.

As has been said, the more defensive characters you have there, the better off you are. Best bet is to send a squishy back with the crew and take two with you to the boss. That leaves Miranda, Garrus, Jacob, Grunt, and Zaeed holding the line. That should be adequate.

In my case, I sent back Grunt and took Mordin and Jack with me. No problems. I had full loyalty and had all the personal upgrades (Geth shields and sniper rifle, Krogan shields and shotgun, etc.)

I never saw anyone "get hit".

Mordin is tasked as the first to die if anyone is going to die on HTL.

During my first run, full paragon and all loyal, none died. Tali ran in heat pipes, Miranda lead 2nd team (also held the line), Thane ran back with crew and Samara with bioshield. Difficulty was veteran I think.

2nd run with full renegade, all loyal except Legion who was sold to the Cerberus (and thus wasn't even with us). Garrus ran in heat pipes, Zaeed lead 2nd team and both got killed. Garrus took rocket to the face (there was some problems with doors), Zaeed got killed after we made thru bioshield section. I didn't have anyone go back with crew so they got killed. Rest survived including Jack (bioshield), Jacob (hold the line). Difficulty was hardcore.

No.. only upgrades and skillset of the various squad mates matters.
