Level: Insane
Frustration Level: Through the Roof
I play an Infiltrator Assassin, I like to play from behind and let the 'Tank' soak the damage. Now my frustration is not with the Characters dieing so easily for I am playing on Insane, it stems from the fact I have to solo most Missions.
Step 1:
Set up points for my guys to stand behind
Step 2:
Pick my own spot
Step 3:
Pretend they can hear me and scream at them (in my head) to
a. Don't just stand up in the assigned spot (taking cover is a good thing to not get shot & die)
b. Stop standing out in the open when I repeatedly tell you to go to another point
c. Stay in the !@#$ing spot I selected for you. This is when they give the same !@#$ you back and run wherever they want
I mean seriously, do they (the ai controlled characters) have to be so dang dumb *sigh*.
Ok, time to go solo another Mission

I don't know how many times I've had to call them back from going gung ho straight into a wall of enemies just cause I found the closest cover. Hopefully in ME3 they'll get the squad AI down. As it is right now it's tolerable, at least it isn't as bad as in ME1.

While I do like the greater autonomy in using their own powers and such, I wish they'd follow Shepard a little closer. I've shot squadmates because they appear out of places they could not possibly have gotten to.

I hate Samara for this reason. But Samara doesn't stand behind the cover ( Jacob does that, but only when Heavy Mechs are firing their gatling gun at him), she runs out into the open, then dies.
They really should have tested the squad AI before putting the game out.
Oh and protip: If you tell your people to get behind cover, then kill all the enemies, they will stay there. Even if you move up and encounter another group of enemies. They won't help unless you call them to you.

Mine did that for awhile, but it turns out that I had biotically thrown some enemies to a place where they didn't die and I just stayed in combat until I died or biotically pulled them back to the map. It was actually Samara that threw them after I pulled, so you might have had the same problem.

I got no problems with Garrus. I tell him to switch to whatever sniper rifle and he procedes to headshot the hell out of everyone and everything in the way.
Generally I push ahead, draw the baddies fire, and then while in cover move my team to where I like them to be.

Nope. I watched Samara go where I told her, shoot some Geth, then run out onto the bridge (Tali recruit mission), and get WTFPWND by the Colossus, 3 geth troopers, a Hunter, and a Flame-throwing Destroyer. At the same time.

My best was, 'Where's Garrus going?!' ... ah crud... over the wall straight into the middle of the gun fire again.
Interesting to note though, I didn't relaize about the fact they try to stay in front of you. That actually may make some sense as to their behaviour. I'll try and remember to really hang back and see if that helps

This spreads the fire in 3 different positions, this should help keep you and your squaddies alive longer.
Definitely max out Grunt's fortification if you haven't. I normally didn't have to "solo" fights, as much as it was only me and Grunt for most of the game, whoever my 3rd squaddie is always dies first.

Send squad down from Kal'Reegar to our side the crates/type stack.
They go down the ramp on the right, walk out towards the first resource crate, then (oddly enough) drop dead because every single Geth has been wailing on them for their act of stupidity.
Meanwhile, I took 6 steps and was in between the 2 spots I'd sent them.

You can't reposition them closeby where you already positioned them, then they won't move or even react - even if you are spamming Q or E. Only way to "re-position" them at almost the same spot is first moving them some other place quite a bit away, then moving them back to the first place. Annoying, but it is what it is.
Quite often the "crouch behind cover but do nothing"-manouver occurs. Solve this by pressing the appropriate Q or E button just a wee bit closer to the cover infront of the squadmate - they will inch forward and actually take cover instead of semi-crouch 1 meter behind cover. And then start firing forward.
If you are setting up your squadmates before any enemies are close and you order them into a position on a cover (the blue positioning marker got slightly ontop of the cover) then it is random which side they take cover... since there are no enemies yet to cover from.
Orders are always reset (null and void) after each cutscene or loading, or trigger event, which means you have to re-issue them.
Pressing C will cancel your orders/positioning and activate 'follow Shepard and kill stuff as best as possible' AI.
Aside from that I've had serious issues with Garrus AI, while Grunt performs outstanding without any micro-managment or positioning (sometimes he get's ahead of himself, but then just pull him back).
edit> I'm also pretty certain the AI is somehow influenced by what weapon you order your squaddie to use, and try to position themselves according to weapon range as an obscure factor.

That said, they do get shot to death by other things at times, but they never screwed the fight by messing in close combat with the Preatorian (unless I had ordered them to stand in a certain position).
Let them on follow Shepard move and you yourself stay out of range of the Praetorian and the AI will run away from it too.
