Ageless perhaps? I just got it for my first time on my 3rd play through, didn't know about it until I saw a weapon list on a goggle search. xD

Starfang, if you have DLC. Axemeter is hard not to miss. Owain's replacement has a very nice bow.

Fade wall: it is not a constant drop from gaxkang

Far Song: best bow in the game, but you may miss it if you decide to save smith's daughter which most goodie gooodie people wil do.

Summer sword: you actually need to kill Ser Cauthrien to get it, and not just let the archers kill you.

(Spoiler) Marjolaine's Recurve. When you do Leliana's companion quest, if you don't loot it after you kill Marjolaine then you won't be able to get back in the house after you leave to look for it. I learned about this bow only after doing that quest. I don't know how I missed it. Maybe a bonehead mistake of not searching her body, or maybe it was in a chest that I lacked the skill to unlock.

Diligence armor set for an Arcana Warrior. I mostly used other heavy armor for my mage like Juggernaut or Wade's Armor. But diligence has some nice armor and willpower bonusses, perfect for an Arcane Warrior.

If you're prone to making deals with demons, it's easy to miss out on the Warden Commander's armor set and the Helm of Honnleath. If you're not thorough with picking up codex entries, you can miss out on the Key to the City in Orzammar.

The Fade Wall is something I always make sure to get - I save before the fight and do it over and over until it drops.

I missed picking up Yusaris the first time I did the Watchguard of the Reaching quest in the Mage's Tower. I killed the Demon in the back corner of the room, but I thought killing him was going to unlock the basement door. So I ran down the steps to the basement but still couldn't get in. When I came back up, I missed seeing the little sack in the corner that contained the item drop. So I completed the quest, and just left my sword laying on the floor.

You can actually win that fight? I thought Cauthrien was simply unkillable to not mess up the story so in the end I just gave up and let myself get captured without a fight.

Marjalaine's bow will be in an unlocked chest in a small room in her house if you decide not to kill her.

It's easy to miss the Key to the City. People love to pretend that they found it on their own, but I think most heard about it, and how to get it, by reading these forums, or via similar word of mouth means.

And while we're on the subject of rings... Lloyd's ring is easy to miss.

The Faulty Amulet is really easy to miss, mainly because it requires that you return to Lothering after visiting Redcliffe, which is something most people don't bother doing.

As for weapons, Austrian's might is easy to miss

You can get the Summer sword  in the fight you have with her in the room before the landsmeet, which is a much easier fight -- she's alone for that one

You can kill Ser Cauthrien when she comes to arrest you and claim the Summer Sword then. It allows you to skip the Fort stuff (where you and Alistair become guards or other people come to rescue you). She's not unkillable, just very difficult.

Warden Tower Shield and Axameter are the easiest to miss. In fact they generally need you to go out of your way to force them to appear.

Fang can be missed easily for city elves, it's probably the second best dagger available.

Or how about blood ring.

Re the summer sword: you get a better Summer Sword if you kill Cauthrien at Howe's estate than if you wait until the Landsmeet. You can also Kill Cauthrien and loot the sword at Howe's estate, then "die" to the archers, and Cauthrien will be back by the Landsmeet (her companions save her like your do you). That way you can get both swords.

You shouldn't win the fith I get the feeling but you can win it, and I have, but it is actually more fun to not win so don't worry.

You're definitely not supposed to win the fight, but easier difficulties it's actually pretty easy to do. On a casual difficulty, I was able to run in with my rogue and just cut people up.

That's not true I got the improved sword both times. Mabey the drop just isn't consistent or on Nightmare you get the better one anway(unless you are playing Nightmare aswell).

Lure her out of the room. The archers won't follow. Kill her by ganging up on her. Then go back and kill all the archers except one, remove your armor then hold everyone, and let the last one kill you.

Alternatively, if you are an AW/BM, just throw up shimmering shield and drop massive AoE stuns. The archers will go down fast, as long as they can't pull off scattershot.
