If you're getting flashing / disappearing mouse pointers and artifacts behind the mouse pointer in-game, try going into Control Panel --> Mouse --> Pointer Options tab and under Visibility, disable 'Display Pointer Trails', 'Hide Pointer While Typing', and/or 'Show Location of Pointer When I Press the CTRL Key' if you have any of them enabled.

I was having all these problems, and disabling these three features made the issues go away.
