First off, the initial dialogs are all must simpler. Since Shepard's dead, no need to reconstruct his face, change professions, nothing.
Then, a special cut scene starts, showing the reapers storming the galaxy, full force. Populations and worlds are devestated. Ships are destroyed. All the characters we knew and loved (NPCs from ME1 and ME2) are husked.
Then a typical arcade-style Game Over screen comes up, with an option to import a non-dead Shepard save.
C'mon, BioWare! You know you wanna!

I actually think it would be pretty damn cool to see a cinematic of some sort that shows what happens if Shepard died in ME2. Maybe it would only "unlock" after you have beat ME3 once. But yeah, hardly a waste of resources.

I still think it would be better if Shepard's body is recovered by a organization called Sirberus, and and brought back to life by the cutting edge mazarus project.
