This has happened to me twice on two different cutscenes. I can't remember one of them right now but here is the one I do remember.
When I recruit Jack and it shows her slide under the door and kill those two Turians. They are not there in the scene. She punches thin air and it reacts like they are there but you can't see them. Has anyone else had this happen?

Hah that happened to me on my second playthrough, very hilarious to watch. Especially when Shep shoots an invisible mercenary behind her.
