can any one tell me exactly where to find every heavy weapons upgrade?

Here is what I know off of the top of my head:
1. Freedom's Progress
2. Zaeed Loyalty mission, if you take Paragon route and save the workers. Otherwise, you get an AR damage upgrade.
3. Blood Pack Installation in Shrike Abyssal. I didn't pick it up but it was there in the mission summary when over.
4. Ratchet Store on Tuchanka
5. Mordin Loyalty quest, at the terminal just before you go to Maelon's room.
There is one more I can't remember. You need 6 of them, in addition to Ordinance Pack N7 armor upgrade to get the M-920 Cain to 200% (two shots).

You can only learn/train 6 upgrades for HW ammo but there are actually 7 upgrades available. The bloodpack N7 mission where you have to follow the beacons does not actually award you (well it didn't for me) the upgrade at the research terminal.
