In the US R is more or less the equivalent of an M rating in video games, which is what both Mass Effects and DA:O got. So steamy stuff is allowed, but not full-frontal nudity (at least, not in any R or M-rated things I've seen) The US equivalent of 18, or R18, would probably be NC-17 or (for video games) AO (short for adults only) but X-rated is a frequently used colloquial term that means more or less the same thing.
Did the theater release version of "Forgetting Sarah Marshall" have the full frontal male nudity in the beginning? I know the unrated DVD did (which is the only movie I know of that has full frontal nudity that is not meant to be sexual).
