I don't think Samara will be back. She told my Shep that though they are close friends, she hopes to never see her again as her code would force her to kill her. Not fun. -100 approval. Samara is one of my favorites.
I don't see Jack coming back. I think working with Shepard is a turning point in her life; either it'll make her worse or a better person. She's ripe for a 'cool cameo'.
Thane is a 50/50. He may be too ill to fight. He might get cured. He might die at the beginning, or anywhere in between.
I can see them having Mordin retire to be with his nephew. He's one of the squadmates that dies the most, so even though he was extremely popular (like Wrex), I can almost be certain that he won't return.
I doubt Samara or Mordin (even if it does make me sad) will be back. The dying LIs are probably a 50/50 shot, depends on if they give us cure quests via DLC (if you don't cure them, they die and are treated as if they died in mission on ME3). But other than those two, every LI and Grunt and Legion have every reason to be back. Kasumi and Zaeed are iffy because they're mercenaries and because they're DLC. I don't even have Zaeed, for instance.
That leaves us with Kaidan/Ashley, Liara, Tali, Garrus, Jacob, Miranda, Grunt, and Legion. Even if you get to keep the two DLC characters, that's 10. Jack and Thane wouldn't be that much of a stretch to keep and still give you your Dirty Dozen.
I hope Garrus sticks around, too. I love listening to his voice. And he's definite BFF material for Shepard.
