Well to knitpick the question a little. The Citadel isn't the best place for a reaper to do a suicide bombing if he is aiming for all out attrition. There are something like 4 million inhabitants on the Citadel. Earth by comparison, has 10 billion. I would imagine he could do more damage by hitting any number of colonies and planets.
Another thing, did you see the size of the Citadel compared to Sovereign? Unless this reaper is an uber giant one, he isn't destroying that thing.
As far as the question, I'm not really a fan of lose, lose, really lose situations, so I'll cheat and put my own spin on the answer. I would probably leave with Tali. There is clearly a better way of explaining to her that you should leave than destroying her emotionally. That's pretty much the bottom right option, like in Tali's trial, that option is only there for Tali haters. The intimidate or charm option would be much better, so I pick that.
As far as letting millions of people die. Does it make me a bad person? Maybe, but I just saved billions from the reapers, and I want my happy ending, I earned it. If a few more people die, I'll just have to deal with it. If I get looked at as a heartless jerk for it so be it.

The Rayya is a civilian vessel. It produces 1/3 of the food for the Migrant Fleet. It's probably got thousands aboard. I dunno what ship Garrus is on, but I bet it's less important.
Garrus is/was a soldier. He knows that his life is forfeit for the right strategic reasons.
