It was created in Garry's Mod. Though there was some color/lighting adjustment done in Photoshop afterwards.

I don't like it... it's not the nostril part its the markings on her face, like she's been cut.. it disturbs and saddens me

impressive work. And, yes, really "Hawt" as well.

You know, off topic, but, I have all the Half Life
games and I have never used gmod.

I think it adds to the overall effect. It's sufficiently alien without losing the prettiness.
