What class are you playing?
I actually found insanity pretty easy when I played.

I always play soldier 

So did I.
Imported ME1 character and used geth shield boost as the extra power, got through the entire game rather easily.

Well i imported a lvl30 ME2 character, with Geth shield boost maxed , still i'm happy that there are no more ****in pyros in the missions i have left

Yes! I hate the pyros.
You'd be walking along minding your own business when all of sudden, one pops out from around a corner and burns you, then Shepard has to stand there dancing.<_<
Luckily I had played through ME2 enough times before my insanity run, so I knew how to deal with pyros.

I just use Miranda as bait and rape the pyro with the revenant while he's baking her :devil:

Tali & Garrus ?

My personal favourite team is Tali & Wrex. I play a vanguard usually so I not only get a team made of win. But I also get a good balance of combat, biotics and tech.

Tali & Ashley, actually. Playing as an Adept, so need good mix of combat/tech to compliment my formidable biotic skills.

When playing engineer, I like to use Tali & Legion.
Just for teh lulz.
When playing any other class I like to use Tali, then either Grunt or Garrus.

Depends on play-style.

For an Adept, Grunt & Mordin compensate for a lack of combat power and tech skills.
For an Infiltrator, Grunt and Miranda provide both up-close power and biotic abilities.
For a Soldier, Jack & Tali provide biotics and tech skills.

Those are the setups I usually use, assuming I want balance.
