The longest I've ever paused in ME to make a decision was which part of the Franklin Colony, in orbit of Watson, to save.

My biggest moral dilemma was during Mordin's quest -- whether to support the idea of curing the genophage or not.

The Paragon option, of course, is primarily objecting to the idea of artificially sterilizing much of the Krogan population, and reviling the horrors that it causes on an individual level (the dead Krogan female, the thousands of stillbirths -- not really murder, although that depends on who you ask I suppose).  But from another perspective, the Paragon option is also advocating what could only cause a huge increase in the Krogan numbers during a time in which their population is still not unified.  This may mean a formidable force with which to fight the Reapers, however, it could have long term consequences for both the Krogan and the rest of the galaxy. 

The Renegade option, on the other hand, entails allowing and even condoning the sort of personal anguish that the genophage is causing.  It also almost certainly means a smaller number of Krogan available to fight against the Reapers.  However, it is also arguably the best solution for the long-term well being of the galaxy and of the Krogan people.  Curing the genophage could allow the different clans to grow unchecked.  The paucity of fertile females would no longer be a concern, and it would destroy the
leverage that Wrex is using to bring the Krogan clans together. 
Introducing the genophage this soon may very well keep the Krogan from achieving a more advanced, more peaceful societal state anytime in the near future, and could very well bring about a new Krogan Rebellion.

I always play Paragon, but this is one option where I chose Renegade.
