Bring loyal Jack with you to the human reaper and she should survive. Jack should only die escorting the crew if she's unloyal, if she's loyal and she still dies while escorting the crew then there's a glitch. Make sure grunt and zaeed are holding the line (and it helps if they are loyal) because they give everyone else a really good chance to surviving too. If you want specifics, I didn't have anyone die this way:

Vents: Loyal Tali/Loyal Legion
Team Leader: Loyal Garrus (you can use loyal/unloyal Miranda too or loyal Jacob)
Biotic: Loyal Samara (you can use Loyal Jack too)
Escort: Loyal Jack (you can use Tali, Mordin or Kasumi too)
Team Leader: Loyal Garrus (you can use loyal/unloyal Miranda too or loyal Jacob)
Human Reaper: Two loyal squad mates that are not Grunt or Zaeed

you should be good to go with this

if everything fails, google is your best friend.

but important! mordin MUST go back. I didnt take him back once and he died on me... lying on the floor while we escaped. Also, take the defence squishies (jack and tali) with you at the final fight, the strong ones can take care of themselves at their last firefight  while you bust the boss fight.

Yeah, DA:O Guide spoilt it for me. Such a good guide - really, really well put together (kudos to Mr Searle for putting so much time and love into it) . Then invested in ME2 one and I was disappoint <_<

Aww well, at least we can share tips and tricks that the guide lacks - ie. last night went through SM and Jack died - she was loyal, but I chose Miri over her in argument - does that cause it? I can't think of anything else that could cause it...or did I miss something obvious...

Do you mean the argument where they got into a biotic fight that Shepard had to break up after their loyalty missions? Because if you side with one, the other will lose their loyalty, but it's possible to regain it through Paragon/Renegade dialogue. Or did you already make her loyal again and she's still dying?

I think I found more advice on the forums than in the strategy guide for ME2, but before Dragon Age, the main reason I bought strategy guides was because my characters tend to get lost. A lot. >.>; And for some reason, when I'm stuck on a battle, the guides often don't help... I actually found my strategy for defeating the thresher maws on the Mass Effect Wiki.

@Collider: Those jumping geth really surprised me at first, and then they were kind of annoying until my aiming improved. I think Garrus and Wrex killed most of them on my first playthrough while my Shepard  just stood there shooting like a madman. xD

I'm given to understand that Mordin can survive holding the line if you have enough "tough guys" still around to cover for him. If you have a loyal Zaeed and Grunt during the hold the line bit it's pretty unlikely, if not impossible, anybody's going to get killed who's loyal.

This is indeed true. I had loyal Zaeed and loyal Grunt as you said, and Mordin survived. And Miranda was unloyal too.

Sided with Jack first run through, lost Miri loyalty but she still made it through SM - Miri wouldn't talk to me rest of game though...ohhhh well ;)  And Jack made it through. Took all the same combos as first RT and Jack died...argument was only difference I could remember.

Well for holding the line, the characters have % to die. The squishier they are (and Jack is squishy) the more likely for them to die. To make the squishies less likely to die, it's good to have loyal Zaeed and Grunt around for holding the line, as well as loyal squad mates in general.

I think I did run through with Collider's formula plenty times, and Jack, Tali, Mordin died either way. So much for compromise. Or maybe 1 in a 1000 players are jinxed like that, lulz.

And so I totally busted my medigels during the human reaper fight, having sent Mordin back, and taking Jack and Tali to the human reaper boss.... because I wanted to save these three darlings.

Everyone is loyal except Jack (because of argument). Second try I sent the Priiiize back with crew  - was worried Mordan might not be tough enough (first Shep sent Jack with crew). Took Garrus/Legion with me first try and Garrus/Thane second. WIll go back to previous save and try again...it is irritating me now...

If Jack is unloyal, then she's even more likely to die...not much around that really. Don't bring unloyal squadmates to the human reaper or they will die. Try bringing squishies like Tali, Mordin, and Kasumi with you to the human reaper so fighters like Garrus/Grunt/Zaeed, etc can protect the other characters while holding the line. The link I gave you should help too.

I put my money on Jack being pivotal in ME3. 
She's such an enigma, never reveals much except her emotional scars and vulnerability.
So save her :)

Miranda wasn't loyal in my first playthrough either (I sided with Jack too), but I think she's just hard to kill. I left Miranda with Grunt holding the line, and she survived. Also, since I don't have Zaeed because he's DLC, and Garrus was fighting the human Reaper with my Shepard, that left only one good hold-the-line squadmate with the team. xD I think every other squadmate dies easier than Miranda.

But if Jack's not loyal, then you won't want to take her with you to the final battle. I think any unloyal squadmate will die after the human Reaper fight when the debris falls on them.

This is true, Kim. Miranda is the least likely squadmate to die. The only surefire way as far as I know is to bring her to the human reaper when she's not loyal.

I always find the "trying to get party members killed" to be a pointless exercise, frankly. Might as well get them all to survive. Why not?

That's the plan for my next playthrough. :) Although I'm not sure what my Shepard's in-game justification for that would be, aside from "Keep your enemies close, and then kick them in front of the giant human Reaper when it shoots at us." Or maybe that's a good enough reason.

And what is worse is that I was grumpy at Bioware for teasing us with 'more DLC' excitement (decent mission expansion? More Garrus? Is it too much to hope..?) and then giving us a stinking visor and some other gear that really isn't much to get worked up about. Was adamant that I would not fall for their evil marketing ploy.

And then I buckled like a squishy and bought it  :whistle:

They are apparently going to announce bigger DLC next week. I hope by bigger they actually mean bigger...they might be announcing the Liara Shadowbroker DLC.
