I had to search the Internet to confirm this and it was actually there.
I dont know if this has been discussed but thought I'd throw it at the community just in case. I have no idea what it means. I tend to agree with the steam community that it is a glitch
I won't steal credit for any of this information. I found a topic about it on the steam forums along with the Utube vid.

Plus he can't be there, people have met with him in person. And he is certainly not as big as the Human Reaper.

It's a bug, unless you really believe that TIM is a giant disembodied head floating in the center of the galaxy.
I lol'ed hard at this!

I suspect Bioware used a technique that Valve used in Half-Life 2. Whenever a character appears on a video screen in that game, it is in fact done live. When a character is talking on screen, the character's model is hidden somewhere in the level, speaking into a camera.
So in this case I would guess the bug is related to spawning and/or clipping issue for TIM's model, or something to that effect

I think this lends credence to the theory that TIM is actually Palmer Eldritch. Our Shep and the entire team are just hallucinating on Can-D the whole way through ME2.
I thought it was suspicious when I picked up the Zeiss logo on the inside of TIM's cold mechanical eyes.
You have to look pretty closely, but he's behind the crack in the ceiling right after Miranda looks over the edge of the platform after the reaper falls.Sphynx118 wrote...

Yeah, I got this bug once too. Probably related to the holo-projection you get of TIM right afterwards.

Yep. Furthur evidence for this can be seen on the Alarai during Tali's loyalty mission. If you activate one of the holo journals, while it's active you can point your crosshairs at a certain part of the wall and see "Quarian scientist" appear on the tooltip. Once the journal playback ends, the scientist disappears.

Yes I suspected this was the method they used after a glitch occured during Tali's loyalty mission. Her father's actor never loaded into the level, so any time her was supposed to appear he was just a disembodied voice. Pretty much ruins the scene. Same thing happended to me in Half-Life 2 once with Dr.Breen.

I had something weird happen playing through the intro on insanity... right before you meet up with Miranda, after fighting those robots that are in that big open room, with the stairs (sorry for vague description)--but on the left end of the hall there is a desk with a recording and a wall safe or something to hack--and once I clicked on the recording, it allowed me to select "talk to Miranda" which I did... I couldn't actually see her, and nothing happened... but she was nowhere to be found after I talked to the Illusive Man the first time. Jacob was waiting by the window, but... no Miranda.
