Does it bother anyone else that Commander Shepard is still COMMANDER Shepard? One would think beating Saren and the Reaper would be worthy of a promotion.

Remember he is working for Cerberus in ME2 not the Alliance it would make no sense to promote him for working with an organization considered to be a terrorist group by practically all of Citadel space.

Well shepard died a couple months after being Saren...They should have promoted him.

Commander Shepard was marked as "Killed in Action". Thus, s/he's not even part of the Alliance Military anymore. Notice how 90% of the people in mass2 now call you simply "Shepard".
I prefer working with Cerberus and/or Spectre status anyway. It has become painfully obvious, in order to pre-empt the reaper Invasion, you need to work outside of Official channels. And you can't do that while wading through red-tape in the Alliance Navy. I definitely like the Shepard-Illusive man relationship too. I hope (even if you destroyed the base) this is something that is continued through to Mass3 (In a big way, not like the Council in Mass1). It's almost an Arms Race of Personality between Shepard and the Illusive Man, each characters foil on the other makes them come out slightly higher than before.

Everyone knows he's lying about those... what was it again? "Reapers"? I mean, they should kick him down in rank for the love of God!

Further, Shepard was brought back to life, and can be reinstated as a Spectre. You keep using the rank -- I'm Commander Shepard and this is my favorite ramen stand on the Citadel, and so forth -- so Shepard must also be reinstated as an Alliance officer. So the question returns: why no promotion?

As Shepard said on the Floatilla. He was never promoted to the rank of Captain. Officially he isn't even in the Alliance military anymore

He should have been promoted after the Citadel event. If not, he should have been posthumously promoted upon his death, its very common.
Though to be fair, everyone in the galaxy will forever know Shep as, Commander Shepard.

If Alliance bureaucracy is anything like that in today's military, his posthumous promotion is still being processed.
That said, even if he left the alliance he would probably be entitled to call himself Commander (Ret'd).

Well, at the start of ME1 the rank was Lieutenant Commander Shepard; a promotion of one grade, would be to Staff Commander Shepard.
Given that Shepard was an N7 and a Marine longer than in the starship command chain, one gathers, after Staff Commander would come Major Shepard. If, ever, the rank was awarded; presumably after the Staff Commander promotion, there was never any need to give further promotions, especially since Shepard's position with the Systems Alliance Navy was ... questionable.
