Jack and Miranda. Jack can give warp ammo to the whole team and use pull often. And Miranda gives everyone a health and damage boost, plus overload and warp is very useful.

Garrus is useful in 90% of situations, due to his weapon choices, ability to incinerate armor and  modest health. He is especially useful against collectors with harbinger, Scions and Praetorians, and when coupled with armor-piercing rounds, makes him much more deadly.
Concussive shot is also useful, and his Overload (especially if Area overload) is very useful-Miranda and Tali also have this power (Tali's is slightly different in function, "Energy Drain"), but have less varied weapon choices and less health.
Tali is useful until you recruit Legion, but he's got all her powers, better weapon choices (Tali can only really fight close-to-mid range, where as legion can fight from afar and at mid), and imo a more useful bonus power, certainly more versatile. However, often I bring Tali along simply because she has more interesting comments, even if Legion is a great character as well, and Garrus and Tali often have amusing conversations due to their history with Shepard.
Grunt is a tank, and is also great in most situations, especially hs high health and fortification-this makes him a huge bullet shield unleashing death. Very useful in most situations. He can fight up close or from a moderate distance.
For Biotic, I usually bring Samara, though Jack is pretty much the same in usefulness levels, and if I'm renegade I bring her along instead. Morinth trumps the two if you get her, though.
I find Zaeed very useful at first as you can get him from the start, that is until you get some better squad mates-he does have a very useful bonus power though, especially for groups of armored enemies, coupled with Disruptor ammo. His personality and comments aren't that good though, so I just forget about him for the most part.
Jacob and Miranda both aren't that useful-Miranda has Overload though, so I say she wins between the two. Sometimes I use Miranda against mechs, and on the first few recruitment missions on insanity, I guess her overload is useful, but she always seems to die.
Thane is useful against Eclipse, as they always tend to have barriers, and quite useful against Collectors as they often do to, because of warp. He is also useful as a sniper, and throw comes in handy, making him useful in a variety of situations. Shredder ammo isn't too useful on insanity though, so you might want to skip him.
Mordin always seems to die when I have him, but he sometimes says some funny stuff, and has some useful abilities (Cyro Blast+Neural Shock). These abilities aren't very userful above Veteran difficulty, though so I baerly use him more then Jacob and Miranda, mainly due to his often funnt comments^_^

If Shepard is Adept or Vanguard, Garrus and Grunt is an ideal squad fighting "organics," paticularly collectors. Against mechs+Geth, Garrus and either Legion/Tali is ideal. A sniper is ideal, especailly if not chosen as proficency on the collector ship, and Thane may come into play due to his warp talents.
If Shepard is a Soldier, Infriltrator or tech, Morinth and either Garrus or Grunt, depending on ranges, are ideal against organics. Against Mechs and Geth, Legion and Garrus are once again the best team-though if you're a tech already, you might want to replaec your tech expert with a biotic to help disperse enemies when their defensive barriers are down. Grunt's ability to fight close range without dying in a few seconds is paticularly useful for a infriltrator.
Sentinel Shepard's ideal squad is Garrus and Grunt, due to the fact a Sniper is needed, and generally Legion's or Tali's tech abilities aren't necessary , though against large numbers of Geth/Mechs, you may want to replace Garrus with a Technician.

Well, that's my guide to squadmates anyway. Works for me.

I can't stand Grunt. His VA pisses me off (the guy is in every freaking video game), and his abilities aren't that diverse if you are playing tactically. He definately has a newbie player feel to him, since his survivability is supposedly so great. When I've tried to take advantage of him with the Claymore though, he dies just as fast as Jacob (on insanity). Zaeed is a much better choice I've found since he can deal with so many different threats.

Miranda + Thane is a hilarious combo for biotic detonations too. Especially the small collector platforms.

Grunt dying usually opens up options for me, he's being shot at more when I'm not. Usually I'm the one being shot at, and Grunt with a Claymore and max fortification, with Krogan Vitality lvl 2 is not a slouch on Insanity either.

Zaeed, Garrus, Thane, all with Incisor Rifles is the most ridiculous thing I've ever seen. Doesn't matter what difficulty it is, they kill everything on screen.

Only time I tend to use Grunt is on his loyalty mission. I use Jacob much more due to preferring pull to concussive shot. I find that each one (apart from grunt in my case), has a time and place were they can really shine. I tend to decide on the squad I take based on the mission I'm doing, the class I'm playing and which abilities they have that can play off each othes and mine to make combo's or simply make life easier. I'll admit that I'm a pull+Unstable warp addict.

I go with Samara and Miranda on my soldier and use Avengers for ARs with squad AP ammo. Reave/warp combos and overload is pretty awesome on insanity.

So far I've used Mordin/Miranda to get me through everything. Will sometimes swap Mordin for Samara.

By no means is it the best team ever, but it works for me.

I dont know Grunt has a couple of things that really make a great squadmate. He can give your team arguably the best ammo power (inferno) and he can rush people and survive quite a bit thanks to his unique rush and armor bar. Finally, he has two decent powers, CS for barriers and stun and fortification for restoring his shields survivability.

Miranda kamikaze? Well, not here. I find her AI to be pretty impressive.

I rarely have problens with squad mates dying,I don't get why some many people complain about this. The only thing I can imagine is that I play with squad-power manual.

I never play with anything other than manual.  Ability use isn't why they die so much, it's the simple fact that they REFUSE to move where you point for them to go quite often. "Moving out" "on my way" with them simply staying perma crouched out in the open happens way too often.  And then once they get hit with something even after they finally did go to cover, they won't auto correct if something knocked them back and the game acts like they are actually behind cover when they aren't.

Are you playing on the PC or 360 version?  I'm playing on the PC and there's numerous annoying bugs.

Grunt makes for good husk bait

Now that I have just about all the upgrades though, I can pretty much use whoever I want and get away with it, even though it's insanity.

As a Soldier I used Miranda and Mordin whenever I could.

When I did Loyalty Missions I would always have Miranda and the person whos loyalty mission it was.

On my engineer insanity run, I almost always brought Mordin along for his incinerate ability. x2.4 damage against armor is absolutely valuable...invaluable...doesn't matter. Having him made it possible to use Dominate very quickly at the beginning of a battle. Send out a combat drone and that's at least 3 enemies occupied right away.

Samara, I suppose would have been a good alternative to Mordin if she got Reave earlier. Against Collectors is where that ability really shines, but by the time she gets it there's only the suicide mission left with them involved. So I tend to only use her at the very end of the game.

Grunt is good as a third teammate due to squad incendiary ammo and for the sheer amount of damage he can take. On the IFF mission he was essentially a combat drone on steroids. I just directed him ahead of me and Mordin so we could incinerate husks as they pummeled him while my drone kept the Scions busy.

That may also be your problem too.  I only give orders like half the time and my squadies do decently and they listen to my orders almost all the time.  I think I only had it happen about two to three times in my  playthroughs and Miranda only did that once.
