So I just reached the werewolf lair for the first time, and reached the Lady of the Forest.
After the conversation with her is complete however, Swiftrunner becomes hostile and attacks the group, preventing me from leaving the area.
I also noticed that during the cutscene, I could not see Swiftrunner talking, all I heard was his voice.
(I caught a glimpse of him while the party was walking towards the Lady of the Forest, he was not moving and kind of crouching in one spot).
Perhaps it has something to do when you first combat encounter him? I'm not sure.
Any help would be great! Thx!

i`ve played through several times and depending on your answers to the lady several outcomes are possible but what your describing hasn`t happened to me and i`m trying not to give anything away so i`ll leave it at that but what i would suggest is trying to load a previous save to see if things turn out different. But i`m pretty sure he should be beside the lady

My responses to the Lady were peace related, as in willing to bring Zath back to the lair for discussion.

ok i don`t know how far you are in game or what level but for me it`s usually toward the last part before i tackle the forest so maybe your coercion skills need to be up a little

I don't think it has to do with the actual conversation.
Because Swiftrunner agrees to what was said, about bringing Zath back to the lair. I'm pretty sure its a technical issue.

I don't think its related to my coercion skills.
I think its a technical issue, because Swiftrunner agrees to with the Lady to bring back Zath to talk..
I don't think that would create a hostile reaction.

I'm having the exact same issue. I had only a few mods installed, all cosmetic changes only; I uninstalled them all and reloaded a prior save from the party camp, replaying the entire Brecilian Forest, only to discover the issue remained unchanged. I suppose there's a chance that one of the mods corrupted the file, and the damage remained even with the mod uninstalled... (Which would suck, because I'm nearing 60 hours (which took two months) on this playthrough).
I also wonder if the problem lies in Return to Ostagar, as the first time I played the game, I had all the same DLC except for RtO, and I had absolutely no problems with the Lady and Swiftrunner. (Of course, I'm no programmer, so I don't even know if/how that would work).
Anyone have any other suggestions?

Are you seeing any lag regarding your combat status? Did you kill the Doorwatcher werewolves? I have had a few instances where durring a cutscene I was in combat because of lag but I have yet to encounter it during the conversation with Witherfang.

Don't know about the OP, but I didn't notice any lag... And I didn't kill the doorwatchers; instead they led me to the Lady of the Forest to negotiate. The only werewolves I've killed were the generic enemies, not the ones you can talk to. Even in the prior confrontations with Swiftrunner, I pressed for peace, fighting only when forced.
The first time I noticed something was wrong was when I talked to the lady and, like the OP mentioned, Swiftrunner was missing from the dialogue shots; I could hear his voice, but the camera was focused on an empty space next to the Lady where Swiftrunner SHOULD have been. He reluctantly agreed to let me go fetch Zathrian and bring him to the Lady, but as soon as the dialogue was over, Swiftrunner (and ONLY Swiftrunner) attacked from his new position off to the side. This is when I entered combat. I couldn't kill Swiftrunner either; his HP hit zero but he stayed up and kept fighting, effectively preventing me from leaving.
