Me. I use Jack and Garrus. Jack gives the whole squad warp ammo and can use pull often, and I just like Garrus. Plus him sniping with tungsten ammo is beast.
Who doesn't use Miranda on every single mission in insanity?

I used to take her on each mission, now i am convinced that garrus (for armor and shields) along with grunt (for armor and tanking) can handle it. and i play on insanity ng+ now, have hard times with shields, but i plan to take AR on collector ship

I use for *almost* ever mission with my VG on insanity. When I'm not using her I'm usually using Grunt or Zaeed. One I can think of is the derelict reaper ship.
Her buffs are nice but since there's virtually no shields or barriers the entire mission I swap her out for Grunt's melee abilities. Add Zaeed's combat prowess to the mix (not to mention the indomitable inferno grenade) and you got a great team. Jack is also a good choice once you strip the armor, her shockwave helps bunches. Plus if you upgraded the shotgun by then (which you should have) then that does a good amount of damage to armor as well. Garrus is another good choice with his sniper and armor piercing rounds...
Besides that mission? The first two parts of the collector base - I almost always have her lead the fire team while I take Thane and Grunt with me to deal with my own sections. Other than that, she's pretty damn useful on insanity.

I play an Adept and I heavily use my powers over my guns. I just love doing explosions. The group I mainly go with is Miranda and Jack until I get Samara and Thane. Then the rest of the game is with Samara and Thane. Area reave and pull field are awesome. And with thane he has Throw Field and Unstable warp. With both of them that is 3 people that can lay down biotic combo explosions plus samara has AR and Thane has Sniper. Its awesome

Well the shuttle apparently only has room for 3 members up to and after the Reaper IFF mission, so it isn't like anyone else was really necessary.
Well, other than the suicide mission.

I use her most of the time. Good character, great abilities. The first time through the game, it's hard not to use her because you don't know what you'll be expecting. It depends on class, too; as an infiltrator she's perfect since she gives you the direct damage buffs against shields and barriers that you lack.

I actually like Jacob as a player in the game.
He gets good shotty pot shots in and when me and other squadie strip the shields insert Pull/shotgun and its good night.
He's not the best, but by no means nerfed, even on insanity. I think they did a great job w/ squad balance

I only use Miranda if i have to. She is a waste of space and breathing air. There are other characters which are more useful in my eyes and work better for my playstyle.

I hardly use Miranda on anything but the required points. But my Shepard is a Sentinel, so I figure I don't really need her that much since I have enough biotic power alone.
On insanity, I was constantly mixing things up to get through parts. Then I got to the collector ship where I spent several hours mix and matching team mates trying to figure out the right team for the situation. I ended up with Thane and Garrus and flying through it like it was nothing finally. Now I use Thane and Garrus for everything, because I just cut through everything like butter with them. Insanity is still hard, but it's now less rape and more strategic fun. Unless of course I'm doing a loyalty mission, then things get sticky, but it's really about balancing the abilities as I'm sure everyone knows.

Miranda has anti-shield and anti-armor abilities, and squad bonuses. She's borderline OP; yet I never really used her. Never cared for her, overall. I'd rather take Thane, or Garrus, or Tali.

She also has warp, operates at nearly her full potential with just 2 abilities maxed, and can max all 3 relevant abilities by level 30.
She doesn't have any defense, but even if you can't be bothered to keep her alive with some micro, AFAIK you retain the squad bonuses even if she dies.

I didn't see a lot of reasons to use Miranda with my Engineer after I got Thane and Samara. I only took her along for some Blue Suns N7-Missions, Geth-fights (not including Talis and Legions loyalty missions - Zaeed was more useful there) and her loyalty mission. Of course, she can strip away defenses, but an Engineer can do that, too (with shorter cooldowns), and for Barriers, Samaras Reave and Thanes Warp are enough - and these Squadmates are more effective otherwise, having access to good weapons and being able to use some nice biotic combos.
For Blood Pack and Animals there are Grunt or Jacob + Mordin
For Eclipse, Collectors and Husks, Samara + Thane are perfect
For Blue Suns and Geth, Zaeed and Miranda are great.
The only squadmates I don't use that often are Tali and Legion (could use them for synthetics, but I am not a big fan of dominate or ai hacking abilities - though having two drones was really fun for Tali's loyalty mission).

I have to admit I didn't really like miranda. I used mordin/grunt against just about everything but heavy synthetics where I would usually use garris/(talia or legion). That's as a sentinal with reave bonus for my insanity run though so rarely used guns to do damage. Yay for grunt being able to tank a little bit.
