 What I am referring to here is strictly the potential deaths of squad mates. I know this post has been made before. I did a slight search but I couldn't find the post I stumbled on earlier about this. I didn't post then, because I had only beaten the game once, with all squad mates alive - I thought it was because i did almost everything in ME2 to bolster my squad.

However, after beating the game again with everything being EXACTLY the same, including squad mates at the end (with one minor exception, I took Jacob with me my 2nd time through instead of Legion), I can safely say the ending result is slightly random? Why else did Mordin die my 2nd time through? He did everything the same way the first time through, but this time he died? Okay I shouldn't say everything was the same, I picked to blow up the base and keep the tech alive the 2nd time through - but this shouldn't affect the outcome, since Mordin died before the station even blew. 

I didn't even go through the game entirely again. I loaded from a save while i was on the collector base because I was unhappy with my final decision to keep the damn human reaper - the smirk on Cerberus's face displeased me to say the least ... Anyway, am I right to assume this is slightly random then?

Short answer: Taking Jacob weakened the "Hold the Line" team, causing the weakest member to die (Mordin). Who else did you take to fight the Reaper? If you took Garrus or Grunt you're leaving the "Hold the Line" team with too little firepower, and people have a chance of dying.

What I find incredibly interesting is if your entire squad dies. If this happens, Shepard lets himself fall...makes sense, but kind of funny after considering what happened in my game.
