...but maybe, I'd save Grunt instead of Thane. I like Thane more, but he's dying anyways, and would probably die soon after the suicide mission
If you could only save 5 of your squad!

Garrus is on 95% of people's lists. Interesting

4. Tali
5. Wrex
Might feel a little sad at leaving Grunt and Jack behind, but Wrex makes up for it.

1. tali (tech, Li from ME 2, no other loyalties besides to me)
2. Garrus (sniper range fighter, been there since 1 and always had my back, no loyalties besides me)
3. jack( great biotic, I'm the only friend she has)
4.Grunt( torn between him and Wrex, but If wrex is dead then Grunt can work his way up the Urdnot clan ranks. I would like to think his first battlemaster would hold a lot of influence in his future)
5.Liara( previous actions have show she cares a great deal for shepard, her information network would be useful)
mordin is old anyway so I would just grab the geno research on my way out.

Question - is this before Shep's inevitable mission to save the galaxy or after? Because if it's after, I think I'd go down with the ship and let someone else take my place. Hell, I'd do that even if we still had to kick the Reapers into touch.

Based on favorites
1) Legion
2) Mordin
3) Grunt
4) Thane
5) Miranda
Based on who would be useful after the mission (No specific order):
Legion - Talking Geth, very important for future relations with Geth. Great fighter, resourceful.
Grunt - Genetically perfect Krogan who can be trusted, strong, intelligent
Miranda - Former Cerberus Operative, genetically perfect, extremely intelligent, resourceful human
Garrus - Really the least unique, but it was between him and Jacob. Good fighter, brings more than Jacob.
Tali - Quarian, access to fleet, kinda has the Grunt situation where she is more at home with you than her race)
Cross-offs (not practical)
Jack - least stable, not unique (my Shep is a powerful biotic), doesn't bring anything new.
Jacob - No negatives, but no real unique positives.
Mordin - Smart, resourceful, useful, but very old and likely to die soon.
Morinth - Already dead from Samara's mission.
Samara - Not sure, but I believe she is not staying with Shep after the mission because of her code
Thane - Powerful, incredibly resourceful, but has a disease and will die soon, so not practical.
Zaeed - He's only in it for the suicide mission, not useful after it, would be useful because he's a feared merc.
Kasumi may replace Garrus for me depending on what she brings to the table.

Tough question! A part of me wants to make judgments based on who I like the most...but that makes things harder than they should be. The next part of me wants to judge based on who has the longest life ahead of them with the most potential to do something with it... but that cuts out so many people given how many of them are actually surprisingly older than you'd expect. Samara's nearing 1000, Mordin's 50, Wrex is getting on in his years, Zaeed's not that young, Thane's getting older and about to die anyway...
That leaves the youngsters: Garrus, Tali, Miranda, Ashley (I killed Kaidan), Liara, Jacob, Jack, Grunt, and I'm not counting Joker in this, nor Kasumi 'cause I don't know squat about her, and Legion, for all his individual personality traits, is still a part of the geth.
However, Wrex is onto something with his people, while Grunt, to me, seems like he'd just cause more destruction. So, Wrex is one of them.
Tali, for all her bravery, was not intended to be on my front line, so I'll save her.
Garrus, 'cause he's awesome.
I'd have said Liara, if she hadn't changed so suddenly in ME2. So in her place... Ashley, I think. She has the potential for good change.
And...Thane, actually. He's got a son, and I think he deserves a chance to make up for some time lost, or at least try and rectify things so that he dies with no regrets later.
So, in sum:
Wrex
Tali
Garrus
Ashley
Thane
Notice how a lot of people aren't saving many humans? Says something, methinks.
Edit: Once again, weird spacing settings are weird.

I'm pretty much just going to pick the old Squad only, not Kaidan he's useless...In his place I want Grunt and in Tali's place I want Legion. Then again, I've already got Garrus for an Infiltrator plus Legion is a Geth, so they could just build another Legion. Oh what the heck, I'll throw Samara in there too.

Legion, Grunt, Tali, Mordin and Miranda.
Without any one or two of those five, the galaxy goes up in flames.
