Possible bugs?

So I had two scenarios where I think I came across possible bugs.  I know I KNOW Sten is suppose to call you out upon coming to Haven for not getting to the business.  Well my last play through he did not what so ever.  So now I'm wondering if he's going to spring it on me later.  I already completed his quest so I'm thinking to may also play into it.

The other scenario is I was on the dwarf town.  I've already completed the circle of magi tower (i did not save the mages).  I was going to smuggle the lyraium potion for the one dwarf in the dirty party of town.  Anyways I look all through out the circle tower thinking he may have survived because of the story.  I go all the way back up to the top to find Uldred back alive.  In his big red beast form he charges me.  And I'll tell you this much he gave me a WAY better fight than last time.  SO much so that he killed both Leillanna and Sten.  He would have killed Shale had Shale not hid behind him the whole time.  Shale had litterally a sliver of life.  I usually hit for about 130 but i was only hitting him for about 60.  Anyways it was weird to see him back alive.  I even went back down and came back up to see if he'd be back again but I guess after being killed the second time he decided he didn't want none lol

I fought a resurrected (revived?) Uldred too--also after NOT saving the mages--when I went back up to the tower to finish looting. (Hey, that's what rogues DO!)
What I have noticed is that the game often takes quite awhile to register that a quest-connected enemy is actually dead, and that you have to stand there and wait until the journal/codex update shows up on screen before the game knows the quest is complete...or else you will see bugs. When I've gone and talked to NPCs after that, for example, I've been reassigned the quest I just completed.
IIRC, after you defeat Uldred, you get whisked down to the ground floor to give Greagoir the good news almost immediately. So probably what happened to both of us was that the transfer happened before the quest completion got posted to our journals, and therefore the game didn't "realize" that Uldred was really dead. After the second fight, though, you just stay put at the top of the tower, so the journal has time to update...and that should be why Uldred doesn't respawn again.
