Shepard can still die even if you do all 11 loyalty missions, you just have to do some of them incorrectly to lose loyalty.

Final Survivors: Joker and remaining crew (Shepard DEAD)

Loyals: Jack, Jacob, Grunt, Garrus, Mordin, Legion, Thane
Non-loyals: Tali, Samara, Miranda (Zaeed dead already)
Normandy Upgrades: None

-Side with Jack during the Miranda/Jack fight
-Hand over the evidence and traumatize Tali for Tali's loyalty mission
-Scare Morinth off when you meet her for Samara's loyalty by selecting all the wrong answers
-Kill Zaeed at the end of his loyalty mission by saving workers but leaving him in the fire he starts
-Bring Tali and Legion to fight the Oculus (so that Thane dies in the engine room)

Vents - Jacob (dies)
Fire Team Leader - Any
Biotic - Samara, non-loyal (bring Miranda and Garrus, Garrus will die)
Second Fire Team Leader - Grunt (dies)
Crew escort - Tali (dies, remaining crew survives)
Final boss squadmates - Miranda and Samara (both die)
Distraction Team - Mordin (dies)

Deaths:
Zaeed - Died in a fire
Jack - No Shield Upgrade, fried
Legion - No Armor Upgrade, incinerated
Thane - No Weapon Upgrade, impaled
Jacob - Rocket to the face
Garrus - Taken by seeker swarm
Grunt - Shot while closing second door
Tali - Died escorting the crew
Miranda - Died at final boss
Samara - Died at final boss
Mordin - Died holding the line by himself
Shepard - Died because he can't do pull-ups to save his life

Man this is gold! Its because i already did two Loyalty Missions but i really wanna see what happens if Shepard dies so i was kinda worried haha.
