The first time we played through ME2, we used an imported character from ME1. We played full out Paragon as a soldier. We did every sidequest we could find, and every loyalty-mission. No problems.
We then started a plus game with the same character, deciding to up the difficulty a little and go full out renegade.
But this is where the trouble starts. We now only have a few sidemissions, Samara's loyalty-mission and locate the Reaper IFF and onwards story-missions left.
And despite having tried to be a b*****d all the way, we only have almost four bars full of renegade. At this point in the game on our previous run-through, we had maxed out paragon about four missions ago.
The result is that we cannot get Morinth, because we don't get any options when trying, and we almost exclusively have either neutral or paragon options when talking to people.

Any helpful tips or explanations?

Oh, and one more thing:
Why is it that in so many conversations you can only choose neutral/paragon or neutral/renegade in ME2?

I think because the individual morality has to be high nough, having max paragon doesnt make affect any Renegade options, unless both are high enough for their own option. Morinth choice needs Max paragon OR renegade. Then you choose the option you want after she tries to dominate you, then i think the paragon is 'suprise' you choose.

Is that's the right answer... idk what the question is completely :P

We are Legion.  There are many of us in this body.

I had the same problem with my renegade playthrough.  I think I was able to come back and get her with 4 full bars of renegade.

I sense here influences of very special ring...(My precioussssss).

And I never reach more than two renegade bars (that was my record achievement) however some dialogs (despite paragon looking) can give renegade points (like question about Virmire victim (upper right answer will give +4 renegade points)).

Fill out your class skill that increases your paragon/renegade by a large %

"We" is us ;) (wife and husband)

And the question was, to put it simply, why don't we get renegade options when we have almost four full bars of renegade?

I guess we'll just have to try to do every side mission we can stumble across and hope that it is enough.

I personally haven't done a new game + yet (still have lots of saves from ME1 to import). However, many other people have said that NG+ is likely bugged for some conversation options (like Morinth) making them impossible. I have no idea if this is true or not, as I have not experienced it myself.

Do you have your 100% renegade/paragon class skill trained?

If its not actually bugged, then I'm sorry I can't really help you. I do know there are more than enough points possible in the game since I just finished a playthrough where I had both paragon and renegade conversation options available for every single conversation in the game.

New Game Plus was not planned to be there in the first place. Then they gave in to the fans and put it in. However, seeing how one can get to level 30 in 1 playthourgh, before going to the suicide mission, NG+ really seems unnecessary. In ME1 it was required to reach level 60. But, for me, it turned out that an imported character with multiple "careers" didn't carry over properly (somehow it came out paragon instead of renegade and all choices were messed up). So I made a new ME1 character, cheated him up to level 60 right away, added some skill points and made a perfect run. And it carried over to ME2 just fine.

So I advise to drop NG+ all along.

i had the same problem. I came back and tried samara loyalty quest at several points in the game, reloading after i failed. I even had over 4 full renegade bars the last time I tried it, but it still didn't work.

I also wasn't able to resolve the legion/tali conflict with renegade (over 4 full bars) and permanently lost tali's loyalty. I was trying to be pretty renegade, but somehow ended up with like 2 bars of paragon.

FYI you get bonuses to the number of paragon/renegade points you earn based on your background. Ruthless/Spacer is renegade bonus, Earthborn/war hero is paragon bonus.

Thanks for lots of good input!

Think we'll try playing renegade with another character import from ME1.
